摘要
对９２例（１０８膝）老年退变性膝关节病患者施行了关节镜检查。于术前根据疼痛、肿胀、行走困难、关节摩擦音、活动受限及Ｘ线片改变程度的综合评分法进行临床分级。１０８膝中，退变性膝关节病轻度１２膝，中度及重度各４８膝。关节镜术中，对软骨退变程度依照Ｏｇｉｌｖｉｅ－Ｈａｒｒｉｓ法分级。关节镜及组织学观察结果显示：老年性退变膝的特征是软骨与滑膜的病变。关节镜分级与临床分级具有较高的符合率（８３．３％）；与Ｘ线片改变程度的符合率较低（４８．１％），说明综合评分对于退变膝的分级较Ｘ线片分级更为合理。
Abstract Arthroscopic examinations were performed in 92 patients(108 knees) aged 60 to 72 years with degenerative arthritis of the knee.Clinical grade was comprehensively evaluated before examination in terms of pain,swelling,walking disability,friction sound,limitation of motion and change in radiography.According to the scores,the 108 knees were classified as mild (12 knees),moderate(48 knees) and severe degenerative knees(48 knees).Arthroscopic evaluation on cartilage degeneration was graded by Ogilvie-Harris method.Arthroscopic and histologic observation showed that the characteristics of the degenerative knee in the elderly were the cartilage change and synovitis.There was high correlation (83.3%) between the clinical grade and arthroscopic evaluation,but a low correlation(48.1%) between the radiographic change and the arthroscopic evaluation.The results suggest that the comprehensive evaluation is more reasonable than radiography for grading of degenerative knee.
