摘要
目的提高闭合性十二指肠损伤早期确诊率和选择适合的手术方法.方法对本院1987～2004年手术治疗的21例十二指肠损伤病人临床资料进行回顾性分析,报道我院采用三管减压引流法应用于15例十二指肠损伤手术的治疗经验.结果术后十二指肠瘘2例,腹腔内脓肿2例,胰瘘1例,肺部感染1例,多脏器功能衰竭2例.治愈19例,死亡2例.结论十二指肠损伤的早期诊断和合理术式应用是非常重要的,采用三管减压引流法治疗十二指肠损伤简便、易行,十二指肠内减压充分,效果好.
Objective To improve the early diagnosis rate of duodenal injury and choose suitable surgical procedures.Methods The clinical data of 21 patients with duodenal injury subject to surgical treatments from 1987 to 2004 were retrospectively analyze. The treatment experience on 15 patients with duodenal injury receiving drainge by three-tube pressure depression.Results Nineteen cases were cured and 2 died. Postoperative complications included: 2 cases of duodenal fistula, 2 of intra-abdominal abscess, 1 of pancreatic fistula, 1 of pulmonary infection, 2 of multiple organ dysfunction syndrome. Conclusion Early diagnosis and suitable methods is very important for duodenal injury. Our method used is simple and rapid with fully decompress to the injured duodenum and satisfactory effects.
