浙江省产超广谱β-内酰胺酶大肠埃希菌和肺炎克雷伯菌的基因分型 被引量:57

Study on genotype distribution of extended-spectrum β-lactamases producing Escherichia coli and Klebsiella pneumoniae in Zhejiang province

摘要 目的了解浙江省产超广谱β内酰胺酶(ESBLs)大肠埃希菌和肺炎克雷伯菌的ESBLs基因型分布。方法对119株实验室保存的1998年9月至1999年6月浙江省各个地区临床分离的产ESBLs大肠埃希菌和肺炎克雷伯菌,采用接合试验,聚合酶链反应(PCR),PCR产物克隆测序等方法明确ESBLs基因型。结果119株菌株中有115株产CT-X-M型ESBLs,阳性率为93.28%,包括88株CTXM14,12株CT-X-M24,6株CTXM22,5株CT-X-M3,2株CT-X-M9及CTXM27,CTXM28、CTX-M-29各1株;9株产SHV型ESBLs,阳性率为7.56%,包括8株SHV12,1株SHV5;未分离到TEM型ESBLs。有9株产2种及2种以上ESBLs,有4株未能确定ESBLs基因型。结论浙江地区产ESBLs大肠埃希菌和肺炎克雷伯菌的基因型以CT-X-M14型为主;CTX-M-29是一种新基因型ESBLs,基因库登录号为AY267213。 Objective To investigate the genotype distribution of extended-spectrum β-lactamases producing Escherichia coli and Klebsiella pneumoniae in Zhejiang province. Methods 119 clinical strains of Escherichia coli and Klebsiella pneumoniae confirmed to produce ESBLs were conserved in our laboratory. All of the strains were collected from 1998 to 1999 in eight cities of Zhejiang province. Plasmid conjugation, PCR, gene cloning and DNA sequencing were carried out for ESBLs genotyping. Results 115 strains (93.28%) produced CTX-M-type ESBLs, including CTX-M-14 (88 strains), CTX-M-24 (12 strains), CTX-M-22 (6 strains), CTX-M-3 (5 strains), CTX-M-9 (2 strains), CTX-M-27 (1 strain), CTX-M-28 (1 strain) and CTX-M-29 (1 strain); 9 strains (7.56%) produced SHV-type ESBLs, including SHV-12 (8 strains) and SHV-5 (1 strain). None of the strains produced TEM-type ESBLs. 9 strains produced two or three kinds ESBLs. 4 strains were negative by PCR. Conclusions CTX-M-type is the most common genotype in plasmid-mediated ESBLs produced by Escherichia coli and Klebsiella pneumoniae in Zhejiang province; It is the first time to report CTX-M-27 and CTX-M-28 in our country; CTX-M-29 is a novel ESBLs and the GenBank accession number is AY267213.
