急性脑外伤后血中AVP和ACTH含量变化的临床意义

摘要 血管加压素(AVP)和促肾上腺皮质激素(ACTH)是体内调节水代谢平衡的重要激素。本文对14例急性脑外伤病人血中AVP、ACTH的含量进行了测定,结果发现:脑外伤后48小时、6~10天血中AVP、ACTH含量明显升高。AVP可增加脑损伤区毛细血管对水的通透性,加重脑水肿。推测AVP/ACTH的比例失调可能是导致严重脑水肿的重要因素。 Arginine vasopressin(AVP)and adrenocortlcotrophic hormone(ACTH) are the important hormones which can regulate the balance of water metabolism in vivo.Thelevels of AVP and ACTH were determined in the blood of 14 patients with acute head injury.At 48 hours and 6~10days after head injury, the levels of AVP and ACTH in the blood increased obviously.AVP could reinforce the permeability of capillary to water in injury areas,and made encephaledema become severe.It is suggested that the disorder of the proportion between AVP and ACTH may be an important factor resulting in severe encephaledema.
