摘要
目的:探讨腹腔镜下肾折叠成形术的有效性及安全性.方法:肾脏巨大积水15例,男9例,女6例,17～58岁,平均38岁,左侧5例,右侧10例.肾盂输尿管移行处狭窄(UPJ)4例,输尿管结石11例,B-us、CT示肾脏巨大积水、肾皮质变薄,KUB+IVU积水肾脏显影延迟.解除梗阻后用1-0可吸收线间断'8'字缝合肾皮质,将肾脏折叠至正常大.结果:15例均获成功,手术时间110～240min,平均150min,术中出血50～100ml,术后住院时间6～10d,平均7.8d,术后1个月经膀胱镜拔出双'J'管,无尿漏发生,随访3～15个月,B-us积水明显减轻或消失,IVU显影示症状较术前明显改善.结论:腹腔镜肾折叠成形术可于处理原发病时同期处理,能减少术后感染及结石复发机会,是安全、有效的微创手术.
Objective:To explore the safety and efficiency of transperitoneal laparoscopic renal plication for giant hydronephrosis.Methods:The reports included 15 cases of giant hydronephrosis(9 males and 6 females),age ranged from 17 to 58 years with mean 38 years,of them 4 cases had ureteropelvic junction(UPJ) obstruction;11 cases had ureteral calcui.The upper urinary tract obstruction was relieved and renal pilcation was performed via laparoscopic surgery.Results:All the 15 cases were completed successfully.The operation time was 110 to 240min(mean 150min),blood loss was 50 to 100ml.The postoperative hospitalization was 6 to 10 days(mean 7.8 days).The doubleJ stent was removed after 1 month.The followup lasted 3 to 15 months.In all the cases,hydroneprosis was relieved and renal function was improved.Conclusions:Transperitioneal laparoscopic renal plication is an effective and safe method for the treatment of giant hydronephrosis.It has the advantage of less inflammatory,less chance of stone recurrence,miniinvasive and dealing with the primary etiology at the same time.
