单纯应用Cage行颈椎椎体间融合对患者神经功能和疼痛症状的改善 被引量:1

摘要 目的:目前Cage在颈椎手术中的应用还存在争议,回顾分析单纯应用Cage治疗对颈椎病患者神经功能和颈肩痛的改善效果。方法:2002-01/2003-01上海第二医科大学附属瑞金医院骨科单纯使用Cage进行前路颈椎间盘切除和融合术治疗的颈椎病患者21例(27个节段),男12例,女9例。应用日本骨科协会颈椎病评分系统(JOA评分)评估手术前后神经功能的变化,应用目测类比评分(VAS)评估颈肩痛的程度,摄片观察融合情况和Cage移位及下沉的发生情况。结果:按意向处理分析,21例患者均进入结果分析,平均随访18个月。①手术后神经功能评分较术前有显著提高(13.7±1.8,9.8±1.5,t=14.11,P<0.01),改善率为40%~75%。②随访时颈肩痛评分较治疗前明显降低3.5±1.2,7.6±0.9,t=17.1,P<0.01。③X射线摄片提示所有患者椎体间融合良好,有1例患者出现Cage的轻度下沉。结论:单纯应用Cage进行颈椎椎体间融合术的患者神经功能和疼痛症状明显改善,椎体间融合率高。 AIM:It is still controversial on the application of Cage in the treatment of c ervical diseases at present.In this study,we reviewed the application of stand alone cage in the treatment of cervical disc disease and evaluated the improveme nt of neural function and cervicobrachialgia. METHODS:Anterior cervical discectomy and fusion using stand alone Cage was us ed to treat 21 patients(12 males and 9 females) with cervical myelopathy(27 segm ents) in the Department of Orthopaedics,Ruijin Hospital, Shanghai Second Medical University from January 2002 to January 2003.Japanese Orthopaedic Association(J OA) scoring was used to assess changes of preoperative and postoperative neural function,visual analogue scale(VAS) scoring to assess the degree of cervicobrach ialgia,and radiography to assess the fusion and Cage's displacement and subsiden ce. RESULTS:According to the intention treatment,all the patients finished the ana lysis for results.Mean follow up period was 18 months.①The postoperative JOA s core was (13.7±1.8) points,significantly higher than the preoperative JOA score [(9.8±1.5) points](t=14.11,P< 0.01),with an improvement rate of 40%to 75%.②T he postoperative VAS score[(3.5±1.2) points] was significantly lower than preop erative VAS score[(7.6±0.9) points](t=17.1,P< 0.01).③All segments showed good intervertebral fusion in the radiography.Slight Cage's subsidence occurred in on e case. CONCLUSION:Stand alone cage used in cervical fusion can obviously improve the neural function and relieve pain with a high rate of fusion.
