摘要
目的:建立广州市学龄儿童的肺功能正常值。方法:应用spiroliteTM303型肺功能仪测定广州市在校健康的6~12岁儿童的肺功能值,测定的指标有14个,取最能反映肺功能的4个参数作为测定指标,即用力肺活量(FVC)、第一秒时间用力呼出气体容量(FE)V、最高呼气流速PEF)和最大呼气中段流速FEF25%((1~75%)。一岁为一个年龄组,共7组,40例/组,总计280例,男女不限,测量体质量和身高,每例测3次,取最好的一次结果记录打印。结果:280例中,男139例,平均身高(135.05±13.42)cm,平均体质量(32.93±7.39)kg,FVC3.40±0.96)L,FEV3.03±0.87)L,PEF((1(6.42±2.32)L,FEF25%(3.58±1.32)L;女141例,平均身高~75%FVC3.22((135.66±12.62)cm,平均体质量(33.06±6.85)kg,±1.07)L,FEV2.88±0.93)L,PEF1((5.84±2.17)L,FEF25%~75%(3.59±1.28)L。单因素分析时FVC,FEV,PEF,FEF25%1~75%4个参数都与体质量相关,多元回归分析时4个参数都与体质量无关,提示属假性相关,表明体质量因素对儿童肺功能影响较小;单因素分析时FVC,FEV,FEF25%1~75%与性别无关(P>0.05),而多元回归分析时FVC,FEV,PEF又与性别相关(P<0.05),提示男女之间的肺功能状态有差1别;无论单因素分析还是多元回归分析,4个参数始终与年龄高度相关(P<0.01)。
AIM:To establish the normal value of pulmonary function for the children of school age in Guangzhou city. METHODS:Fourteen parameters of pulmonary function of healthy 6 to 12 year old school children in Guangzhou city were measured by using spiroliteTM303 pulmonary function test spirometer,4 parameters were selected,including forced vital capacity(FVC),forced expiratory volume in first second(FEV1),peak expiratory flow rate(PEF) and forced expiratory middle flow rate(FEF25% - 75% ),which could the best reflect the pulmonary function.Totally 280 boys or girls were divided into 7 groups with 40 cases in each group according to their ages,1 year as a group separation.The body mass and body height of each case were measured for 3 times,and the best results were recorded and printed. RESULTS:Among the 280 cases,139 were male,the mean body height was(135.05± 13.42) cm,mean body mass was(32.93± 7.39) kg,FVC was(3.40± 0.96) L, FEV1 was(3.03± 0.87) L,PEF was(6.42± 2.32) L,and FEF25% 75% was(3.58± 1.32) L.The other 141 of the 280 cases were female, the mean body height was(135.66± 12.62) cm,mean body mass was(33.06± 6.85) kg, FVC was(3.22± 1.07) L,FEV1 was(2.88± 0.93) L,PEF was(5.84± 2.17) L,and FEF25% 75% was(3.59± 1.28) L.Single factor analysis showed that FVC,FEV1,PEF and FEF25% 75% were all correlated with body mass,while the multiple regression analysis showed that all the 4 parameters had no correlation with body mass,which suggested that it was a false correlation, it was indicated that body mass had little influence on the pulmonary function of children.Single factor analysis showed that FVC,FEV1 and FEF25% - 75% had no relation with gender(P >0.05),but the multiple regression analysis showed that they were correlated with gender(P< 0.05),it was suggested that the status of pulmonary function was different between males and females.Both the single factor analysis and multiple regression analysis indicated that the 4 parameters were correlated with age(P< 0.01). CONCLUSION:The parameters of children's pulmonary function are correlated with age,gender, body height and body mass,while the parameters are not affected by the same factors.Age and body height are the most important factors that influence the results of pulmonary function parameters.
