摘要
目的:研究云克联合慢作用抗风湿药治疗活动性类风湿关节炎的疗效及安全性。方法:64例患者随机分入云克组和对照组,云克组采用云克+甲氨喋呤+柳氮磺吡啶治疗组,对照组采用甲氨喋呤+柳氮磺吡啶治疗,比较观察两组患者压痛关节数、压痛指数、肿胀关节数、肿胀指数、晨僵、握力、休息痛、医生评分、患者评分及血沉、C反应蛋白的改善及不良反应发生的情况。结果:两组患者各项疗效指标较治疗前明显改善(P<0.05);经治疗4周后云克组各项疗效指标改善优于对照组(P<0.05);经治疗12周后云克组压痛指数、肿胀指数、晨僵、握力、休息痛、患者评分及血沉、C反应蛋白(CRP)各疗效指标改善优于对照组(P<0.05),而压痛关节、肿胀关节、肿胀指数、医生评分各疗效指标改善并不优于对照组(P>0.05)。两组均无严重不良反应的发生(χ2=0.034,P=0.854)。结论:云克+甲氨喋呤+柳氮磺吡啶治疗类风湿关节炎安全有效。
AIM:To study the efficacy and safety of combination therapy of yunke (99Tc methylene diphosphonate,99TcMDP)and methotrexate and sulfasalazine for patients with active rheumatoid arthritis(RA). METHODS:Sixty four RA patients were randomized into yunke group(yunke+ methotrexate+ sulfasalazine) and control group(methotrexate+ sulfasalazine).Tender joint count,tender joint index, swollen joint count, swollen joint index, morning stiffness, grip strength,rest pain,patient global assessment,physician global assessment,erythrocyte sedimentation(ESR),C reactive protein(CRP) were observed.Simultaneously, the adverse reactions were recorded. RESULTS:All the indices had been improved significantly after treatment(P< 0.05).After 4 week treatment,the improvements in all the indexes were more obvious in the yunke group as compared with the control group(P< 0.05).Compared with the control group, the yunke group had a superior improvement in the tender joint index, morning stiffness,grip strength, rest pain,patients global assessment,and ESR(P< 0.05),but not in the tender joint count,swollen joint count, swollen joint index and physician global assessment after 12 weeks(P >0.05).None of the severe adverse reaction occurred in the two groups(χ 2=0.034, P=0.854). CONCLUSION:Combination therapy of yunke, methotrexate and sulfasalazine is safe and effective for treatment of RA.
