摘要
目的:观察小腿腓肠神经滋养血管的走行、来源及其与周围组织的吻合关系,设计以腓动脉穿支为血供的逆行岛状皮瓣,进行足踝部软组织缺损的修复。方法:通过选取1 0例成人小腿标本,进行腓肠神经及其伴行血管的显微解剖研究,观测腓肠神经滋养血管的来源、构成及相互吻合关系,在此基础上应用节段性腓动脉穿支为血供的带蒂逆行岛状皮瓣并应用于临床,修复足踝部缺损1 0例。结果:解剖研究表明:小腿腓肠神经全程均有伴行动静脉滋养,行程中不断接受不同平面的穿支加强,并与周围组织形成广泛的吻合。在踝上3~7cm范围内有较大的腓动脉穿支与之吻合,提供血液滋养并藉此为其周围皮肤供血。以此神经血管蒂设计逆行岛状皮瓣修复足部软组织缺损1 0例,全部成活。结论:以腓动脉穿支—腓肠神经滋养血管—皮肤血管网的营养皮瓣,为修复足踝部软组织缺损的较好的方法,且简单易行。
Objective: To study origin, route, and vascular anastomosis of the sural neurovascular, design the sural neurovascular distal island flap nurtured by the perforating branches of the peroneal artery,and repair destroyed soft tissues of ankle. Methods: 10 adult cadaveric leg samples were selected for an anatomic measure study on the sural neurovascular, and the origin route and vascular anastomosis of the sural neurovascular were observed. The sural neurovascular distal island flap was used clinically to repair soft tissues of ankles in10 cases. Results: There is constant vascular anastomosis between the sural neurovascular and peroneal artery. And there is the constant artery and vein supply for the sural nerve and skin around the sural nerve. The flap survived completely in 10 cases. Conclusion: It is comparatively a good and easy method to have the design of the sural neurovascular flap in which soft tissue defect is repaired with the sural neurovascular flap.;
