摘要
目的探讨膀胱非上皮性肿瘤的诊断及治疗。方法对2000～2004年我院收治的5例非上皮性膀胱肿瘤患者的临床资料进行分析。结果膀胱非上皮性肿瘤患者的主要临床表现为血尿、尿频、排尿困难等症状,主要辅助检查为B超、CT、膀胱镜检查及镜下活检。本组5例中,男4例,女1例,年龄6～45岁。经术后病理检查,恶性肿瘤4例,分别为膀胱横纹肌肉瘤2例、膀胱小细胞癌1例、膀胱平滑肌肉瘤1例;良性1例为膀胱嗜铬细胞瘤。5例患者均行膀胱部分切除术,1例术后8个月死亡,其余4例现随访均存活。结论膀胱非上皮性肿瘤临床少见,术前诊断率低。手术是该病的主要治疗方法。良性肿瘤应完整切除,恶性肿瘤应争取广泛切除,结合其病理特点辅助放化疗可提高疗效。
Objective To evaluate diagnosis and treatment of non-epithelial tissue tumor in bladder. Methods The clinical data of 5 patients with non-epithelial tissue tumor in bladder from 2000～2004 in our hospital were analyzed. Results Of 5 patients, 4 were male and 1 was female. The patients’ age ranged from 6～45 years. The clinical manifestations showed painless gross hematuria, frequency of micturition and dysuria. Ultrasonic examination, computed tomography scan, cystoscopy and biopsy were performed in 5 patients. Four cases were of malignant neoplasm with 3 kinds of pathologic types. There were 2 cases of rhabdomyosarcoma, one of leiomyosarcoma and one of small cell carcinoma. One benign tumor was pheochromocytom. All 5 patients underwent the partial cystectomy. In the period of following-up, one patient died in postoperative 8 months, and the rest 4 patients were alive. Conclusion Non-epithelial tissue tumor in bladder is rare in clinic with complicated pathologic types. The rate of comfirmed diagnosis before operation is low. Dipbiopsy undercystoscopy may increase the diagnosis rate. Surgical treatment is the main therapy for non-epithelial tissue tumor in bladder. Because of the aggressive biologic behavior of malignant tumors, they should be identified promptly and treated appropriately. According to the histologic features, radiotherapy and chemotherapy are mandatory in some cases.
