右心声学造影实时三维超声心动图测量犬右室心搏量 被引量:3

Measuring right ventricular stroke volume using contrast real-time three-dimensional echocardiography: an experimental study

摘要 目的:评价实时三维超声心动图(RT3DE)结合右心声学造影技术测量右室心搏量的准确性。方法:应用实时三维超声诊断仪对15只健康杂种犬分别采集无造影、经静脉右心造影并同步经左心耳左心造影的心尖长轴全容积三维数据库,脱机后应用容积分析软件进行后处理分析,分别用2、4、8、16平面法勾画右室内膜面,测量右室舒张末期容积(RVEDV)与收缩末期容积(RVESV),计算右室每搏量(RVSV),同时用16平面法测量左室舒张末期容积(LVEDV)与收缩末期容积(LVESV),计算左室每搏量(LVSV),进行左右每搏量对照分析。结果:运用右心声学造影后,右室内膜面显示更为清晰,可重复性增加。RT3DE有造影2平面、4平面、8平面以及16平面法测得的RVSV与16平面法测得的LVSV之间的相关性分别为0.701,0.800,0.894,0.980。无造影RT3DE上2平面、4平面、8平面以及16平面法测得的RVSV与16平面法测得的LVSV之间的相关性分别为0.690,0.758,0.852,0.904。相同平面法比较显示RT3DE有造影的相关性都要高于RT3DE无造影。LVSV与RT3DE心尖长轴2平面法测量有造影的RVSV比较有显著性差异(P<0.05),与4、8和16平面法比较无显著性差异(P>0.05);而与2和4平面法测量无造影的RVSV比较有显著性差异(P<0.05),与8和16平面法比较无显著性差异(P>0.05)。 Objective: To evaluate the accuracy of measuring right ventricular stroke volume by contrast real-time three-dimensional echocardiography. Methods: Fifteen healthy mongrel dogs were examined by real-time three-dimensional echocardiography(RT3DE). Apical 'full volume' 3D data of RV, RV and LV with contrast were acquired. RV volume was measured off-line using 2, 4, 8 and 16-plane method with the software of workstation and RV stroke volume(RVSV) was calculated. LV volume calculated only using 16-plane method. The results of RVSV measured by different methods were compared with left ventricular stroke volume(LVSV), which served as a reference standard. Results: RV endocardial border became distinct after using contrast, which offered better reproducibility of RV volume measurement. Good correlation was showed between RVSV(obtained by 2, 4, 8 and 16-plane methods separately with no contrast) and LVSV(r=0.690, 0.758, 0.852 and 0.904 respectively). But there was a higher correlation between RVSV which obtained through 2, 4, 8 and 16-plane methods with contrast and LVSV(r=0.701, 0.800, 0.894 and 0.980 respectively). There was difference between LVSV and RVSV in 2 and 4-plane methods with no contrast(P<0.05), but no difference between LVSV and RVSV in 8 and 16-plane methods with no contrast(P>0.05). Whereas, there was difference between LVSV and RVSV only in 2-plane method with contrast(P<0.05), but no difference between LVSV and RVSV in 4, 8 and 16-plane methods with contrast(P>0.05). Conclusion: Contrast RT3DE can improve the accuracy and reproducibility of measurement of right ventricular stroke volume.
