血同型半胱氨酸在2型糖尿病肾病中的作用观察 被引量:4

摘要 目的:探讨血浆总同型半胱氨酸水平(Hcy)与2型糖尿病肾病的关系。方法:应用高效液相色谱仪和荧光检测器测定39例2型糖尿病肾病变组和32例2型糖尿病无肾病变组的血浆总Hcy水平。并对糖尿病肾病患者应用胰岛素治疗 4 周后复查血浆总Hcy。结果:2型糖尿病肾病变组血浆Hcy,明显高于无肾病变组,胰岛素治疗 4 周后无明显改变。Logistic回归分析提示高Hcy是糖尿病肾病的独立危险因子。结论:2型糖尿病肾病变组血浆Hcy明显高于2型糖尿病无肾病变组,胰岛素治疗不能降低2型糖尿病肾病变组血浆Hcy。高Hcy是糖尿病肾病的独立危险因子。 Objective:To investigate the relationship between the plasma total homocysteine level and nephropathy in type 2 diabetes.Methods:71 patients'homocysteine concentration was determined by HPLC method.39 of them were of type 2 diabetes nephropathy,and cured with insulin for 4 weeks.Results:The homocysteine concentration of type 2 diabetes nephropathy patients was 19.1±9.2μ mol/L,significantly higher than that of type 2 diabetes non-nephropathy patients,and the change was not obvious after 4 weeks curing of insulin.Multiple linear regresson analysis showed that homocysteine correlated with type 2 diabetes nephropathy imdependently.Conclusion:The homocysteine concentration of type 2 diabetes nephropathy patients was significantly higher than that of type 2 diabetes non-nephropathy patients,and the change was not obvious after 4 weeks curing of insulin.The high level of Hcy is independent risk for nephropathy in type 2 diabetes mellitus.
