吡喹酮治疗人体肝、肺包虫病的观察

摘要 25例肝、肺包虫病患者口服吡喹酮治疗7天后手术摘除包虫囊肿,利用高效液相色谱仪,检测了包虫囊内药物浓度和血药浓度,证实吡喹酮确可进入包虫囊内,但其浓度很低,肝包虫囊内浓度高于肺包虫,血药浓度明显高于囊内浓度。应用透射和扫描电镜观察发现,治疗后生发层皮层细胞均有不同程度的空泡增多,线粒体、高尔基复合体,粗面内质网、糖原减少,出现较多溶酶体和髓样结构等;原头节呈外翻型,吸盘凸出,后部短缩,体表呈粗糙颗粒状或带状,散在分布泡状结构;少数原头节变化更显著。原头节活力鉴定提示,治疗后肝包虫囊内原头节着色率(62.7%)高于肺包虫(36.6%)。 The concentration of praziquantel was assayed and the changes of fine structures of the greminal layer and the protoscoleces of hydatid cysts were studied from 25 patients with lung or liver hydatid disease who underwent surgery after 7-day medication with a total dose of 300mg praziquantel therapy. Our results definitely indicate that praziquantel can be found in the hydatid cysts wall and its fluid but it is very low. The drug level in cyst walls and fluid in the pulmonary group is lower than that in the hepatic group, although the level in blood is similar for both groups. This difference co-incided with the better results in treatment of hepatid hydatid disease. In the treated group both in the hepatic and pulmonary cysts the greminal layer showed that the number of vacules increased but decreased in mitochondria, golgi complex, rough endoplasmic reticulum and glycogen, more lysosome and myelinoid body were observed in the cytoplasm of tegument cells. Most of the protoscoleces evaginated and suckers protruded.The tegument surface of the rear part was obviously damaged. It is confirmed, therefore, that praziquantel is of protoscolicidal effect in the treatment of hydatid disease and the mortality of the protoscoleces in hepatic cysts is higher than that in the pulmonary cysts in the treated group.
