摘要
本文报告我院十年间在内镜下治疗１８９例，２７５颗上消化道大息肉（息肉＞１０ｍｍ）。采用方法有单纯性圈套套切法；圈套套切加息肉基底部注射硬化剂法及特大息肉的分次切除法。本组除１例胃穿孔，２例术后大出血外，无其他严重并发症。本文讨论了镜下摘除上消化道大息肉的术前准备，术中注意事项、并发症及术后处理。讨论了上消化道大息肉内镜下摘除的必要性。
From Fcb, 1978 to Fcb.1990,one hunderd and eighty nine patients with upper gastrointestinal big polyps were diagnosed and excised by endoscopy in our hospital.We have treated 189 cases (275 polyps)by (1) resection at one precedure using high frequency snare or by (2) resection at two precedured by which roots of polyps were injected on 2～3 sites with 0. 5ml of 99. 9% ethanol or 1% Aethoxysklerol,then polyps were resected completcly using a high frequency snare. If the polyps was bigger or leafy,it could be resected for two or three times. No serious complications had happened,but 8 cases exhibited hemorrhage after polypectomy,and were stopped with endoscopy. The pathological diagnosis were as follows: imflammatary and hyperplasia polyps (77. 8%), adcnoma (14. 5%), Peutz-Jegher's syndrom (4. 4%). Only one case of adenoma had carcinomatous change.The authors suggested that not only smaller polyps in upper gastrointestinal tract can be excised completelly with endoscopy but also larger polyps can be managed safely and effectively under endoscopy.
