摘要
我院1973～1987年收治结肠癌320例,其中左侧235例,占73.44％,包括乙状结肠174例(74.04％),降结肠35例(14.89％),脾曲24例(10.21％),多部位结肠2例(0.85％)。235例中并发急性完全性梗阻79例,占33.62％。本文将79例左侧结肠癌引起急性完全性梗阻病例作一分析,并对其治疗和术式问题加以讨论。临床资料本组79例中男52例,女27例,年龄17～79岁,其中40岁以上占81％(64/79)。肿瘤部位乙状结肠60例(75.95％),降结肠11例(13.92％),脾曲8例(10.13％)。病理类型腺癌57例(72.15％),未分化癌11例(13.93％),其它类型11例(13.93％)。治疗方法及结果本组79例,手术切除率为86.1％(68/79)。其中14例作Ⅰ期病变肠段切除吻合术,6例发生吻合瘘(43％),
During 1973～1987, 79 cases of cancerous obstruction of the left colon were treated. Among them, 14 cases were treated by primary resection with anastomosis, anastomotic leakage occur- red in 6 and 5 were dead. Three-stage procedure was performed in 25 cases, in which the fecal stream was diverted by transverse colostomy. In the second operation, the diseased areas could be resected only in 20 patients. 6 cases with advanced cancer were undertaken colostomy only. In other cases, 19 cases with modified Hartmann's procedure and 15 cases by extended right he- micolectomy with ileorectal or low ileosigmoid anastomosis, give satisfactory results. The latter two procedures were recommended by the author.
