体外震波粉碎胆结石的实验研究 被引量:3

EXPERIMENTAL STUDIES ON FRAGMENTATION OF GALLSTONES BY EXTRACORPOREAL SHOCK WAVES

摘要 作者应用最大输出功率1 MW的EDAP LT-01体外震波碎石机,研究体外震波对在模拟胆囊和离体胆囊内不同大小和成分的胆结石的粉碎效果,以及震波对胆囊组织的损伤。结果提示,在本实验条件下,色素性结石比胆固醇性结石容易粉碎。对直径11mm以下的胆囊结石和肝内胆管结石,治疗功率越高,碎石效果越好。碎石效果并不随冲击频率的增加而相应地增加,使用每秒1.25至2.5次的冲击频率碎石60min较为合适。震波可引起离体胆囊上皮的损伤,故应将震波焦点放在胆囊粘膜层以上。碎石过程的良好监控是成功的关键。 An EDAP LT-01 lithotripter with, a maximum output of 1MW was used to study its efficiency for fragmentation of gallstones of different size and composition in gallbladder models as well as in vitro human gallbladder, and to evaluate tissue damage induced by the extraoorporeal shook waves. Results suggest that under these experimental conditions, pigment stones were more easily fragmented than cholesterol stones. The higher the output power used, the bettor the result for gallbladder stones less than 11mm in diameter and intrahepatio bile duct stones. The lithotriptio effect, however, did not increase in correspondence to inoreaed rate of firing. We think that 1.25 to 2.5 firings per second for 60 minutes would be appropriate. Shook waves may induce epithelial damage in removed human gallbladder, so the shock-wave focus should be positioned a slight distance away from the mucosal surface of the gallbladder. The key to success is careful monitoring throughout the lithotriptio procedure.
