摘要
从综合ICU 227例危重病例中选择有病原学诊断的51例感染病例进行菌群分析,发现综合ICU感染以G^-杆菌和真菌为主。感染部位以肺部最多(76.5%),其次为泌尿系感染及败血症(各31.4%)。肺部感染主要为绿脓杆菌(71.8%)和醋酸钙不动杆菌(64.1%);两种或两种以上细菌感染33.3%,合并真菌感染30.8%。泌尿系感染以真菌为主(93.8%),多与其它部位感染同时存在。败血症以绿脓杆菌和真菌为主(各37.5%)。院内、外感染组的感染部位、数目和菌种间均无显著差异(P>0.05)。提示:以G^-杆菌和真菌感染为主的多部位,多种病原菌混合感染是综合ICU感染的主要特征。为有效控制感染,在未获得病原学诊断前,应选择广谱抗生素,必要时合用抗真菌治疗。
The pathogenic results were analyzed in 51 pa-tients who were selected from 227 critically pa-tients in general intensive care unit(ICU).It wasfound that gram-negative bacilli and fungi werepredomonant pathogens in the ICU.In these 51patients with pathogenic evidences,pulmonary in-fections were the most commonly seen infections(76.5 %).Next to pulmonary infection were urinarytract infections(91.4%)and septicaemia(31.4%).In pulmonary infections,predomonant pathogenswere pseudomonas aeruginosa(71.8%)andacinetobacter calcoacetlcus(64.1%),the infectionswith two or more than two different kinds ofpathogens were 33.3%,and concurrent fungal in-fections were 30.8%.In urinary tract infections,fungi were the most important pathogens(93.8%)and usually were concucuent with infections ofother sites.Pseudomonas aeruginosa(37.5%)andfungi(37.5%)were the predominant pathogens ofsepticemia.There were no significant differences asto the sites,number and pathogens of the infec-tions between the group of nosocomoal infectionand that of primary infection(P>0.05).It is sug-gested that the infection in the general ICU ischaracteriged by mixed infections of multiplepathogens on multiple sites with gram-negativebacilli and fungi being predominant.Broad spec-trum antibiotics which are effective againstgram-negative bacilli should be employed beforethe pathogenic evidence is available.Anti-fungalagents may be administered simultaneously as indi-cated.
