摘要
在32个灌注红色乳胶、墨汁和手血管铸型标本上,解剖观察了示指背侧皮瓣的血供来源、分布和吻合。结果显示:以第一掌骨近端尺侧筋膜血管神经为蒂的示指背岛状筋膜皮瓣,即含有第一掌背动脉(多数)、头静脉的属支示指背静脉和桡神经浅支的分支,又有多个来源形成的纵横交错的筋膜血管网。皮瓣切取不受第一掌背动脉类型的影响,手术操作简单,经临床15例应用证明安全可靠,值得推广应用。
The origin,distribution and anastomoses of the blood vessels nourishing the island dorsalflap of index finger were dissected and observed in 26 hand specimens.The results showed thatthe island dorsal flap could be designed with the fascia pedicle which is on the ulnar side and cov-ers the proximal part of the first metacarpal bone.The flap contains the first dorsal metacarpalartery,the branches of cephalic vein and nerve fibers from the superficial branch of radial nerve,as well as a multi-origin vascular net to supply blood to it.15 cases of successful clinical applica-tions fully proved the advantages of the flap being safe,easy to operate and is not affected by thetype of the first dorsal metacarpal artery,and showed a broad prospects for further clinic use.
