成都地区绝经后妇女脊椎压缩性骨折的患病情况及骨密度变化 被引量:4

摘要 目的 本文旨在探明绝经后妇女脊椎压缩性骨折的高发部位、患病率随年龄变化的规律以及BMD、BMC 等指标的变化规律。方法 以居住在成都地区具有正式户口年龄在50~89 岁的健康人群为调查对象,进行随机抽样。对每名受试者进行胸腰椎侧位摄片,若楔形指数(椎体前缘/后缘)≤0.8 即诊断为压缩性骨折,同时采用DXA 对全部受试者腰椎正位及髋部的BMD 进行测定。计算不同部位及不同年龄段的脊椎压缩性骨折的患病情况。配对后对骨折组和对照组的BMD 及其他各项指标进行t检验。结果 胸椎压缩性骨折患病率为8.49% ,占脊椎压缩性骨折的76.1% ,其中T6、T7、T11、T12的患病率高,共占胸椎压缩性骨折的69.14% ;腰椎压缩性骨折的患病率相对较低,占脊椎压缩性骨折的23.9% ,主要集中于L1、L2,占腰椎压缩性骨折的93.55% 。脊椎压缩性骨折的患病率随年龄的增加而逐渐上升。骨折组腰椎BMD、BMC 明显低于对照组(P< 0.01),但二者的面积无显著性差异(P>0.05)。结论 绝经后妇女随着年龄的增加脊椎压缩性骨折的患病率逐渐升高。胸椎压缩性骨折的患病率高于腰椎,主要发生于T6、T7、T11、T12。 Objective\ To study the high risk sites and prevalence rate of vertebral compression fracture and changes with age,BMD and BMC in postmenopausal women. Method\ Based on the results of X\|ray,we calculated the prevalence rate of vertebral compression fracture in different sites and age groups. By dual energy X\|ray absorptiometry,we compared the results of BMD,BMC and AREA in patients to those in the controls with matched t test. Result\ The prevalence rate of compression fracture increased with age. The prevalence rate of thoracic vertebral compression fracture was 8.49%,comprising 76.1% of all vertebral compression fractures,in which the fractures of T\-6,T\-7,T 11 and T 12 were predominant. The prevalence rate of lumbar vertebrae compression fracture was relatively lower,comprising 23.9% of all vertebral compression fractures,mainly occuring in L\-1 and L\-2. The BMD and BMC of patients were significantly lower than those of the controls ( P <0.01),but the difference in AREA between them was not significant. Conclusions\ The prevalence rate of vertebral compression fracture increases with age. The prevalence rate of thoracic vertebral compression fracture is ralatively higher than that of lumbar vertebrae. The prevalence rates of T 6,T 7,T 11 and T 12 are relatively high. The lumbar vertebral compression fracture mainly occurs in L\-1 and L\-2. The reduction in BMD and BMC is an important risk factor and we can predict the risk of vertebral compression fracture by BMD and BMC measurements. So we can effectively prevent vertebral compression fracture by reducing the loss of bone mass. There is not significant difference in AREA between the patients and the controls, so we cannot predict vertebral compression fracture by AREA. If we measure the BMD of lateral vertebrae,we may get a more meaningful conclusion.
