自动调节通气频率和潮气量通气模式的临床应用研究 被引量:13

摘要 目的 客观评价临床应用适应性支持通气(ASV)的可能性。方法 采用自身前后对照的研究方法,选择行有创通气的病人11例,分别给予常规机械通气方式和ASV。常规通气方式的参数设置由医师根据病情决定,ASV的参数设置包括三个参数,即病人体重(WT)、分钟通气量百分比(MV%)和高压报警限。MV%常规设为100%(相当于每公斤体重100ml/min)。FiO_2和呼气末正压(PEEP)与常规方式相同。两种方式各持续运用1h后采集数据。对比观察两种通气方式下的潮气量、呼吸频率、气道压、心率和血压。结果 与常规通气方式相比,ASV方式下的潮气量和呼吸频率无显著性变化,对气道压、心率和血压无明显影响(P>0.05)。结论 ASV根据病人呼吸力学状况调节通气参数,能获得与常规通气近似的呼吸形式,是一种操作简便、有效而安全的通气模式。 Objective To assese clinical implication of adaptive support ventilation (ASV) ,a computerized mode which select tidal volume and respiratory rate based on lung mechanics and preset minute ventilation, in intubated patients.Methods Conventional mechanical ventilation (CMV) mode and ASV were randomly used in eleven intubated patients.Tidal volume .respiratory rate .airway pressure, heart rate and arterial blood pressure were measured in CMV and ASV.Results Tidal volume,respiratory rate , airway pressure, heart rate and arterial blood pressure were no significant differences in CMV and ASV.Conclusions Satisfactory breathing patterns can be determined by a computerized mode which selected tidal volume and respiratory rate based on lung mechanics and preset mi u'te ventilation.
