小剂量脂质体前列腺素E_1对急性肺损伤犬心肺功能的改善作用 被引量:4

Effects of low dosage of liposomal prostaglandin E1 on hemodynamics, oxygen metabolism and lung mechanics in a canine model of oleic acid induced acute lung injury

摘要 目的 研究脂质体前列腺素E_1(lip-PGE_1)对油酸诱发急性肺损伤(OA-ALI)犬血流动力学、氧动力学和呼吸力学的影响。方法 12只杂种犬经戊巴比妥钠静脉麻醉和经口气管插管,经右股静脉括入Swan-Ganz浮漂导管,经右股动脉插入动脉导管。经Swan-Ganz管CVP端注入油酸0.09ml/kg建立ALI模型。将大随机分成对照组(C组):机械通气+静脉注射0.9%盐水;lip-PGE_1组:机械通气+静脉注射0.025μg·kg^(-1)·min^(-1)lip-PGE_1。测定致伤前(基础)、治疗开始(0min)和治疗后60min的血流动力学、氧动力学和呼吸力学数据。结果 lip-PGE_1组平均动脉压和平均肺动脉压60min较0min明显降低(P<0.05),心输出量显著升高(P<0.05),动脉氧分压和氧合指数显著升高(P<0.01)。lip-PGE_1组氧供60min较0min组和C组60min显著升高(P<0.01),氧耗60min较0min明显升高(P<0.05)。lip-PGE_1治疗60min后呼吸力学变化不显著。结论 小剂量lip-PGE_1能显著增加OA-ALI犬的心输出量,降低平均肺动脉压,改善氧合指数、增加氧供和氧耗。 Objective To test whether administration of lower dosage of iiposomal prostaglandin E1 (lip-PGE1 ) can improve cardiopulmonary function in a canine model of oleic acid induced acute lung injury (OA-ALI). Methods Twelve pentobarbilal-anesthetized dogs were induced to devolep AL1 by injecting oleic acid (0.09 ml/kg) into the right atrium via CVP port of PA (pulmonary artery) catheter, and then divided into two groups (control and lip-PGE1) of six dogs each. After AL1 models were established, the control group was simply ventilated ( pretreated with normal saline) and lip-PGE1 group was treated with mechanical ventilation (MV) plus pretreatment with lip-PGE1 intravenously at dosage of 0.025 μg·kg-1·min-1 for 60 min.Measurements of hemodynamics,gas exchange,and lung mechanics were performed at baseline, the beginning of therapy (0 min), and 60 min after therapy. Results Lip-PGE1 significantly redived MAP and mPAP(P<0.05)as well as increased cardiac output( P<0.05) .Arterial oxygen partial pressure,oxygenation index were noticeably improved( P< 0.01, P<0.05 respeclively) 60 min after lip-PGE1 intervention. There were an increase of DO2 and VO2( P<0.05) and reduced venous admixture (Qs/Qt) after 60 min of lip-PGE1 pretreatmenT( P<0.01). Lip-PGE1 had no influence on parameters of lung mechanics except a slight increment of VT. Conclusions lawer dasage (0.025 μg·kg-1·min-1 ) of lip-PGE| significantly improved cardiac performance and oxygenation in OA-ALI canines within 60 min.
