摘要
目的:探讨社区精神分裂症患者的治疗依从性差与社会心理因素的关系,为社区康复提供依据。方法:对社区康复的符合精神分裂症诊断标准且资料完整的795例患者按治疗依从与否进行自然分组,研究组(n=177)为治疗依从性差者,对照组(n=618)为较好的治疗依从者;调查观察2年。比较两组社会心理因素的差异及与病情的相关,分析研究组治疗依从性差的影响因素。结果:两组间存在着社会心理因素的差异:研究组的家庭气氛较差、家人对患者关心程度低、与邻居及同事的关系不好、较常得不到经济及情感上的支持、家庭经济较差(P<0.05～0.01);两组现有病情和社会功能与家人对患者的态度、家庭经济、在经济及情感上的支持、家庭气氛呈不同程度的正相关(r=0.084～0.273,P<0.05～0.01),社会影响与各社会心理因素呈负相关(r=0.091～0.285,P<0.05～0.01),且均以研究组的相关性更明显;家人对患者的态度差、家庭气氛不良、得不到经济和情感上的支持、病程长是治疗依从性差的主要易感因素。结论:社区康复的精神分裂症患者治疗依从性差与不良的社会心理因素密切相关,具有女性构成比高、教育程度低和病程长的特点。必须重视精神分裂症的社区防治和康复,积极改善社会心理因素,对提高治疗依从性、促进康复有重要意义。
AIM:To investigate the correlation between therapeutic dependence and psychosocial factors in community schizophrenic patients, so as to provide reference for community rehabilitation. METHODS:Totally 795 cases, who were in accordance with the diagnosis of schizophrenia and finished the study in community,were naturally divided into study group(n=177,patients with bad therapeutic dependence) and control group(n=618,patients with good therapeutic dependence) according to the therapeutic dependence, they were investigated and observed for two years.The differences of psychosocial factors and their relations with condition of ill were compared,and the influencing factors of bad therapeutic dependence in the study group were analyzed. RESULTS:There were differences in psychosocial factors between the two groups: patients in the study group had worse family environment,fewer care from family members,worse relationship with neighbor and colleagues,more lack of economic and emotional support,worse family income(P< 0.05 to 0.01).The condition of illness and social function had positive correlations to different extent with family attitude, family economy, support of economy and emotion, and family environment in both groups(r=0.084 to 0.273,P< 0.05 to 0.01),while social influence was negatively correlated with all the psychosocial factors(r=0.091 to 0.285,P< 0.05 to 0.01),and the correlations were more obvious in the study group.The main susceptible factors for bad therapeutic dependence were bad family attitude,family air,lack of economic and emotional supports and long disease course. CONCLUSION:The bad therapeutic dependence of community rehabilitative schizophrenic patients is closely correlated with their psychosocial factors,and has the features of higher constituent ratio in females,lower educational level and long disease course.Importance should be attached to the community treatment,prevention and rehabilitation.It is of great significance to actively improve the psychosocial factors so as to increase the therapeutic dependence and promote rehabilitation.
