摘要
目的探讨射频消融左侧心外膜旁道的方法学及有效性、安全性。方法选择32例左侧旁道患者经心内膜途径标测和消融失败者,经冠状静脉窦内标测到旁道电位或A蛳V(V蛳A)最近、融合处,应用能量或温控消融电极,分别在冠状窦内或穿刺房间隔在二尖瓣环左房侧射频消融。结果32例中29例成功,成功率为90.6%,其中2例经穿刺房间隔在左心房侧消融成功;例失败。结论左侧心外膜旁道多数可经冠状静脉窦和左心房有效消融。
Objective To discuss the methods?efficacy and safety of radiofrequency catheter ablation of left-sided subepicardial accessory pathways. Methods The ablation catheter was introduced into the coronary sinus or left atrium by transseptal technique and the energy(15 ~ 25w) or temperature (60℃ ~ 65℃) applied at the site of recorded accessory potential or the earliest activation site of atrium/ventricular to 32 patients undergone unsuccessful endocardial mapping and catheter ablation. Results 29 of 32 patients had successful ablation, 2 pathways were ablated at left atrium by transseptal technique, the successful rate was 90.6%. Conclusions Radiofrequency ablation via coronary sinus and left atrium is an effective therapeutic method to eradicate subpicardial accessory pathways.
