摘要
通过对30只成人尸体手的显微解剖,发现中指背侧也可形成岛状皮瓣,其营养血管为第二掌背动脉。该动脉在鼻烟窝基底尺侧起自桡动脉腕背支,走行在第二骨间背侧肌表面的筋膜层中,在掌骨底远侧及掌指关节水平均与掌侧血管有吻合。该动脉供血可达中指背基节中远1/3交界处,两侧可达侧中线。皮瓣的静脉回流由两条伴行静脉完成,皮瓣神经为走行在皮下脂肪中的桡神经浅支。临床应用3例,均获成功。
This paper have carried out the microdissection of 30 cadoveric palma manuses.The results showed that in addition to fore finger,on the middle finger dorsal surface there is a axial skin flap supplied blood by the second dorsal metacarpal artery that originates from the radisl artery dorsal carpal branch at the ulner basal part of naspharyngeal fornix.This artery goes into the fascial space on the surface of the second intermetacarpal dorsal muscle.It anastomoses with palmar artery at wrist and on the leyel of metacarpophalangeal.It supplied blood to the one third junction between intermediate part and distal part of the middle finger coxa.Every artery is accompanied with two veins.The middle finger dorsal flap is dominated by the surperficial branch of radial nerve going in subcutaneous fat with the second dorsal metacarpal artery roughly.Three patients have been operated.All skin flap was alive.
