摘要
目的:探讨颈椎退行性变病发生与血流量变化的相关性,为颈椎病的早期治疗提供参考。方法:用32只家兔,进行实验性血流量改变和颈椎应力改变对颈椎组织结构影响的比较研究,高分子右旋糖酐复制血瘀模型,让兔低头改变颈部应力,进行颈部组织血流量测定、X线分析,并结合关节突、关节囊、韧带及其止点病理形态的光镜、电镜观察。结果:(低头)应力引起颈部组织退行性变过程中,存在着组织的充血,水肿等血瘀变化。血流量下降,局部缺氧状况,组织的充血、水肿等,诱发成纤维细胞向成骨细胞转化,从而加速和加重应力引起颈部组织损伤,是致病因素之一。局部血流量改变可以影响应力引起的颈部骨-韧带组织退行性改变,骨组织和软组织改变是不同步的。结论:颈椎局部血流量下降是颈椎病发病的重要一环,并加速和加重发病。
Objective To explore the effects of the changes of regional blood flow(rBF) and cervical stress on the recessive abnormality of cervical spine. Methods This experiment was based on 32 rabbits. After having changed the stress and strain distribution on the rabbits cervical vertebrae by letting them bow their heads 2 hours every time, twice a day for three months. The rBF was measured with hydrogen clearance technique, and the pathological change of the cervical vertebrae, intervertebral discs, the capsular ligment, the neck muscles and the attachments of the capsular ligment to bones were inspected by light microscope and electron microscope. Results The increase of stress and strain could cause degeneration of the cervical vertebrae, intervertebral discs, capsular ligaments and neck muscles. The ischemia could lead to the morphological abnormalities of the cervical spine. Conclusions The abnormalities of the soft tissues around the cervical vertebral bones, which was associated with the swelling induced by ischemia, play an important part in the development of the cervical spondylosis.
