摘要
目的 探讨乳腺叶状肿瘤超声声像图特征及其病理基础。方法 应用彩色多普勒超声检查 8例 ( 12个病灶 )乳腺叶状肿瘤病患者 ,观察其二维及血流显像图表现 ,并与病理对照分析。结果 乳腺叶状肿瘤具有体积大小范围变化大、分叶或角状、内部中低不均匀回声而后方回声衰减、有包膜但边界不清、内部血供丰富为主等特征性声像图表现 ,本组血流信号检出率为 10 0 % ,平均收缩期最大流速 ( 13 .8± 4.0 )cm s ,平均RI 0 .77± 0 .0 8;主要为大量间质异形细胞成分并膨胀性生长、变性坏死及包膜侵犯等乳腺叶状肿瘤的病理基础与上述特征相对应。结论 乳腺叶状肿瘤的确诊目前主要靠病理诊断 ,超声误诊率虽较高 ,但其具有一定的较典型图像特征 。
Objective To investigate the sonographic features of phyllodes tumor of the Breast and its basis of pathology.Methods Phyllodes tumor of Breast were examined in 8 cases (12 lesions) by Ultrasound and Color Doppler flow image。Ultrasonographic findings (size,margin,shape,echogenicity and color Doppler blood patterns) of lesions were recorded ,meanwhile,the relative pathologic were studied.Results Breast Phyllodes tumor often were showed as the bulk had greater change generally, lobulated or angular mass , lower middle echo of the internal tumor with hard light strips and lateral calcification, unclear margin with capsule and lesions with irregular posterior shadowing in image of Ultrasonography; The rate of color Doppler blood flow showed was 100%,Vmax was (13.8±4.0) cm/s,RI was 0.77±0.08;Pathologic findings of breast Phyllodes tumor indicated that there were a large number of interstitial cells and a few of tumor cells.They grew fast in a short time with denatured necrosis and capsule destroyed.Conclusion Now Pathological diagnosis was most depended in breast Phyllodes tumor,.Misdiagnosis rate shall be further improved in Ultrasonograpgy . But part of imaging of breast phyllodes tumor was typical, Corresponding with color Doppler flow Ultrasonograpgy can be helpful in the differentiation of the breast phyllodes tumor from breast benign tumor and the other malignant tumors.
