摘要
目的 探讨脑囊虫病不典型表现 ,旨在提高对其认识。资料与方法 搜集大囊型脑囊虫病 18例进行回顾性分析。 12例行CT检查 ,5例行MRI检查 ,1例同时接受CT和MRI检查。所有患者血和脑脊液酶联免疫吸附试验 (ELISA)、间接血凝试验 (IHA)及囊虫皮试均 2项以上阳性。结果 18例中 ,单发囊性病变 9例 ,多发 9例 ,共发现 4 5个囊性病灶 ;脑实质内 30个 ,蛛网膜下腔 11个 ,脑室系统 4个。病灶呈圆形及类圆形 ,短径 0 .4~ 6 .0cm ,平均2 .2cm ,长径 0 .6~ 8.0cm ,平均 3.1cm。 9个 (2 0 % )囊肿内可见头节。 3个囊肿周围见水肿 ,9个 (9/ 2 1,4 2 .9% )囊肿或囊壁有强化。结论 大囊型脑囊虫病 ,脑实质型、脑膜型、脑室型及混合型均可见 ,需与脑脓肿、蛛网膜囊肿、表皮样囊肿及各种原因所致的脑软化灶鉴别。
Objective To improve the knowledge of atypical encephalo cysticercosis.Materials and Methods 18 cases with encephalo cysticercosis manifestated as larger cysts were retrospective analysed. Of 18 cases, 12 patients had CT scan, 5 cases had MRI, and 1 case had both CT and MRI examinations. Two or more of all the tests that included blood and CSF ELISA, IHA and derm test were positive in all patients.Results Solitary cystic structures of CSF-equivalent signal intensity were found in 9 cases and multiple cystic structures in 9 cases. There were 45 cysts detected in all with 30 parenchymal cysts, 11 subarachnoid cysts and 4 intraventricular cysts. The shapes of the cysts were round and para round with short diameters 0.4~6 cm, mean 2.2 cm, and long diameters 0.6~8 cm, mean 3.1 cm. Scolices were seen in 9 cysts (20%). Surrounding focal edema was visualized in 3 cysts and ringlike contrast enhancement was seen in 9 cysts (9/21, 42.9%).Conclusion Cysticercosis demonstrated as larger cysts were seen in parenchymal, meningeal, intraventricular and mixed form. The differential diagnosis includes arachnoid cyst, epidermoid, abscess and cerebromalacia due to various causes.
