研究目的 探讨小于胎龄儿、适于胎龄儿及孕母血生长激素水平和生后10～56月小于胎龄儿的生长发育情况。 研究方法 病例对照研究。用放射免疫分析法测定孕母及新生儿脐血生长激素水平。观察小于胎龄儿生后10～56月的生长发育情况。 研究结果 平均生长激素水平,新生儿为15.87±13.42ng/ml,孕母3.85±2.18ng/ml。小于胎龄儿16.13±13.71ng/ml(服可乐定前),24.80 ±17.50ng/ml(服可乐定后);适于胎龄儿16.47±17.47ng/ml(服可乐定前),22.30±19.11ng/ml(服可乐定后)。生后第1、3、 5天生长激素分别为17.00±14.84,19.40±18.08和11.87±10.58ng/ml。出院56例小于胎龄儿随访,41.07％(身长)、35.71％(体重)、26.79％(头围)仍小于第十百分位数;而对照组仅14.28％,1.79％和16.07％在第十百分位数以下。 结论 新生儿生长激素浓度呈高水平,明显高于孕母水平。出生后前5天血中生长激素持续呈高水平。小于胎龄儿和适于胎龄儿生长激素水平无差异。小于胎龄儿生长发育落后于适于胎龄儿。
Objectives To search for the growth hormone (GH) levels in small-for-gestational age (SGA) infants,appropriate-for-gestational age (AGA) in fants and pregnant women. The growth and development of SGAinfants at the age of 10～56 months were investigated. Methods Cace-control research. The growth normone levels of pregnant women and newborn infantswere determined by radioimmunoassay. The growth and development of SGA infants at the age of 10～56months were investigated. Results Mean serum GH levels, newborn infants is 15.87±13.42ng/ml, pregnant women is. 3.85±2.18ng/ml. SGA newborn infants are 16.13±13.71 (before intake colonidine) and 24.80±17.50ng/ml (after in-take colonidine). AGA newborn infants are 16.47±17.47 (before intake colonidine) and 22.30±19.11ng/ml(after intake colonidine). The mean serum GH levels of the 1st, 3rd and 5th day after birth are 17,00±14.84, 19.40±18.08 and 11.87±10.58ng/ml respectively. 56 SGA infant: were followed up at age of 10～56 months compared with 56 AGA infants, 41.07% (lenth), 35.71% (Weight), 26.79% (circumference)were still below tbe 10th percentile in SGA infants, while 14,28%, 1.79% and 16.07% below the 10th pe-rcentile respectively in AGA infants. Conclusion The levels of serum GH are at high level in newborn infants which is significantly higherthan the levels of pregnant women. GH concentration persists at a high level in the first five days. Thereis no significant difference between the SGA and AGA newborn infants. The growth development of SGAinfants is poorer than that of AGA infants.
