摘要 目的 观察颈部皮下CO2 充气对腔镜下甲状腺手术动脉血气的影响。 方法  2 0 0 2年 6月~ 2 0 0 3年 3月 7例气管插管全麻下行腔镜下甲状腺手术 ,皮下充入CO2 压力 6mmHg ,监测动脉血pH、PaCO2 变化。 结果 充气后 5minPaCO2升高 ,pH值下降 ,15minPaCO2 达到高峰 (44 33± 0 97)mmHg ,pH值最低 (7 36± 0 0 2 ) ,充气后各时间段PaCO2 和pH与充气前比有显著差异 (P <0 0 5 ) ;30min后PaCO2 开始下降 ,pH值上升 ,1hPaCO2 、pH值达到稳定水平 ;放气后PaCO2 、pH值接近充气前水平。 结论 皮下CO2 充气对腔镜下甲状腺手术动脉血PaCO2 、pH值有影响 ,但仍在正常范围。 Objective To observe the effects of subcutaneous carbon dioxide insufflation on arterial blood-gas values during endoscopic thyroid operations. Methods Seven patients underwent endoscopic thyroid operations under general anesthesia with endotracheal intubation. The pressure of CO 2 insufflation was set at 6 mm Hg. The arterial pH value and PaCO 2 were monitored. Results The arterial PaCO 2 began to increase and the pH value decreased 5 min after the insufflation. The PaCO 2 reached the peak (44 33±0 97 mm Hg) and the pH value fell to the lowest level (7 36±0 02) 15 min after the insufflation. The PaCO 2 and pH value showed remarkable difference before and after the insufflation ( P <0 05). The PaCO 2 began to decrease and the pH value went up 30 min after the insufflation and they reached at stable levels 1 h after the insufflation. After the deflation, the PaCO 2 and pH value went to the levels before the insufflation. Conclusions Subcutaneous carbon dioxide insufflation has effects upon the arterial PaCO 2 and pH value during endoscopic thyroid operations, but these effects are still within the normal ranges.
