基质金属蛋白酶-9在川崎病外周血表达及其在冠状动脉损伤中的临床意义 被引量:6

摘要 目的:观察基质金属蛋白酶-9(MMP鄄9)在川崎病不同时期表达水平,探讨其在冠状动脉(冠脉)损伤中的作用。方法:选择明确诊断为川崎病患儿27例(无冠脉损伤组10例,冠脉损伤组17例),按病程分为急性期、亚急性期和恢复期;另以年龄相仿的10例败血症患儿、10例正常儿童为对照组。分别应用明胶酶谱法、酶联免疫吸附法检测血清MMP鄄9的活性和蛋白浓度,半定量RT鄄PCR法检测外周血白细胞MMP鄄9mRNA表达水平。结果:所有川崎病患儿急性期血清MMP鄄9活性和蛋白浓度显著高于发热对照组和正常对照组(P<0.01),而到亚急性期、恢复期则依次明显降低(P<0.01)。与无冠脉损伤组比较,冠脉损伤组患儿急性期MMP鄄9活性和蛋白浓度升高更明显(P<0.05)。川崎病患儿急性期外周血白细胞MMP鄄9mRNA表达明显增强(P<0.05),血清MMP鄄9蛋白浓度与循环白细胞总数正相关(r=0.480,P<0.05)。结论:MMP鄄9在川崎病急性期尤其伴冠脉损伤时表达明显升高;MMP鄄9可能参与了川崎病血管炎尤其是冠脉扩张损伤的病理过程。 Objective: To investigate the potential role of matrix metalloproteinase 9(MMP-9) in the pathogenesis of coronary artery lesions in patients with Kawasaki disease(KD). Methods: 27 patients with KD, including 10 patients who had no coronary artery lesions(CALs) and 17 patients who had CALs, as well as age-matched febrile(n = 10) and afebrile(n = 10) controls patients were studied. Gelatin zymography and enzyme-linked immunosorbent assay(ELISA) were used to detect the enzyme activity and the total protein levels of MMP-9 in the sera. Expression of MMP-9 mRNA in the leucocytes was studied using reverse transcription-polymerase chain reaction. Results: The serum enzyme activity and total protein levels of MMP-9 in the acute phase of KD were significantly higher than those in the febrile and afebrile controls(P < 0.01) and decreased obviously from the subacute through the convalescent phases(P < 0.01). Moreover, the serum total protein levels of MMP-9 and MMP-9 enzyme activity in the acute phase of KD with CALs were significantly higher than those in KD without CALs(P < 0.05). The expression of MMP-9 mRNA in the leucocytes in the acute phase of KD was significantly elevated, compared with the febrile and afebrile controls(P < 0.05). There was a significantly positive correlation between the serum total protein levels of MMP-9 in the acute phase of KD and the circulating leucocyte counts(r = 0.480, P < 0.05). Conclusions: Over-expressions of MMP-9 in the acute phase of KD may be involved in the formation of coronary artery lesions and pathogenesis of KD.
