摘要
采用系列单克隆抗体(McAbs)和聚合酶链反应(PCR)技术研究35例淋巴细胞白血病免疫表型与免疫球蛋白重链(IgH)和T细胞受体(TCR)γ、δ基因重排。结果表明,20例表达不同分化阶段B细胞表面标记,其中18例发生IgH基因重排,8例TCRγ基因重排.2例TCR_δ基因缺失。8例表达不同分化阶段T细胞表面标记,均检测到TCRγ和、或TCR_δ基因重排,无IgH基因重排。3例同时表达T、B细胞表面抗原者均发生IgH和TCRγ基因双重重排。4例缺乏表面抗原表达者中3例发生TCRδ或TCRγ基因重排。2例完全缓解病例经PCR扩增证明存在残留白血病细胞。
The immunophenotype and the rearrangements of immunogiobulin heavy chain (Igh) and T cell
receptors (TCR) gamma (γ), delta (δ) genes of 35 cases of lymphocytic leukemias were studied by
using a panel of monoclonal antibodies(McAbs) and polymerase chain reaction (PCR), respectively.
The results suggested that there were 20 patients whose leukemic cells expressed B cell surface markers
of different defferentiation phases, gene rearrangement of IgH were detected in 18 cases of them, and
of the 8 cases of TCR_γ gene rearrangement, 2 cases had a deletion of TCR_δ gene. All 8 cases whose
cells expressed T cell surface antigens had TCR_γ and/or TCR_δ gene rearrangement, but none had IgH
gene rearrangement. The 3 cases whose cells coexpressed T and B cells surface antigens had double-
rearrangements of IgH and TCR_γ genes. 3/4 cases whose cells fail in expression of surface markers had
TCR_γ or TCR_δ gene rearrangement and 2 cases who achieved complete remission were detected residual
leukemic cells by using PCR gene amplification.
