摘要
在30例病人中对比了传统的经验公式法和脉图积分法测量的平均动脉压(SAMP)的准确性。结果显示,经验公式法与心导管测量的SAMP相关良好(r=0.89),但有显著低估(P<0.001),脉图积分法与心导管测量的SAMP高度相关(r=0.93)且差异无显著性,脉图积分法为无创性估测SAMP提供了简便可靠的新方法。
Systemic arterial mean pressure(SAMP)is an important
hemodynamic parameter in clinical cardiology.In clinical practice,however,
this parameter has traditionally been estimated by an empirical formula.To
develop(?)new method for noninvasively measuring SAMP,radial arterial
electropulsograms,brachial arterial systolic(BASP)and diastolic(BADP)
pressures ard the aortic pressure curves were recorded almost simultaneously in
30 patients undergoing left heart catheterization.The data were processed by a
computer system and the following indexes were obtained:the electropulsogram-
derived pulse pressure-time integral(Ap),the systemic arterial mean
pressure calculated by an eleetropulsographic integral method(EAMP),the
catheterization-derived pulse pressure-time integral(Ac),and the aortic
systolic(AOSP),diastolic(AODP)and mean(AOMP)pressures.The
result showed that there were high correlations between Ap and Ac(r=0.94,
SEE=0.21kPa·s),between BASP and AOSP(r=0.96,SEE=0.62kPa),and
between BADP and AODP(r=0.86,SEE=0.86kPa).Although SAMP
estimated by the traditional empirical formula correlated highly with AOMP
(r=0.89,SEE=0.78kPa),there was a significant underestimation(P<0.001).
On the other hand,EAMP correlated closely with AOMP(r=0.93,SEE=
0.58kPa),and there was no significant difference between the two means.
We conclude that the electropulsographic integral method developed in this
study offers a simple and reliable approach to the noninvasive monitoring of
SAMP.
