摘要
背景:直流电场能促进脊髓再生,但伤后6h置入电剌激器的疗效比伤后立即置入电剌激器的疗效为差,可能与脊髓损伤后出现脊髓水肿有关。目的:探讨直流电场与汉防己甲素联合应用治疗完全性急性脊髓损伤的疗效。设计:随机对照的实验。单位:海南省人民医院骨病外科。材料:实验在海南省人民医院完成。33只中国家犬,体质量10~12kg,犬龄1.5~2岁,由海南省动物中心提供。干预:将33只家犬随机分成3组,用AllenWD法致脊髓完全损伤。A组为对照组;B组汉防己甲素治疗组;C组脊髓损伤2h静滴汉防己甲素,脊髓损伤6h再置入电刺激器组。主要观察指标:伤后各组1,2,3个月神经功能、皮质体感诱发电位、神经元数量、神经元截面积和内氏体密度恢复情况。结果:B,C组神经功能早期恢复,P1潜伏期明显缩短,波幅明显升高,神经元计数明显增多,神经元截面积增大及内氏体密度深犤B组1,2,3个月时各组数值分别为(27.65±1.19),(20.48±1.02),(17.45±1.08)ms;(724±52),(877±41),(1035±50)nV,(37.7±3.4),(45.6±3.2),(52.5±2.9)个;(154.96±6.01),(181.40±6.86),(237.47±7.38)μm2,(0.4694±0.0261),(0.5996±0.0302),(0.6351±0.0326)pixel,C组则分别为(21.25±0.83),(14.63±0.90),(13.35±0.84)ms;(915±61),(1146±36),(1262±49)nV,
BACKGROUND:The direct current field can promote the regeneration of the spinal cord.But the result of implanting the stimulator in body 6 hours after injury is not as good as implanting it immediately. This is possibly due to the spinal edema after injury. OBJECTIVE:To study the effect of direct current stimulation together with tetrandrine(Tet) for the acute complete injury of spinal cord. DESIGN:A randomized and controlled study. SETTING:Department of Orthopedics,People's Hospital of Hainan Province. MATERIALS:The experiment was done in the People's Hospital of Hainan Province.Thirty-three healthy Chinese hybrid dogs,weighing 10-12 kg,aged 1.5-2 years,were provided by the Animal Center of Hainan Province. INTERVENTIONS:Thirty-three dogs were divided randomly into three groups and injured the spinal cord completely with Allen WD method.Group A was the control group.Group B was treated with Tet,Group C was given Tet through intravenous drip 2 hours after injury and the electric stimulator implanting 6 hours after injury. MAIN OUTCOME MEASURES:The recovery of nerve functions, cortical somatosensory evoked potential(CSEP),quantity and cross sectional area of neurons,and density of Negri's bodies at the periods of one,two and three months after injury. RESULTS:The recovery of nerve functions of Group B and C was earlier,the P1 latency obviously shortened,the amplitude clearly higher,the neurons evidently increased,the cross sectional area of neurons enlarged,and the density of Negri's bodies deepened. [Group B,at the periods of 1,2,and 3 months,the numerical value was respectively(27.65±1.19),(20.48 ±1.02),(17.45±1.08)ms;(724±52),(877±41),(1 035±50)nV,(37.7±3.4),(45.6±3.2),(52.5±2.9);(154.96±6.01),(181.40 ±6.86),(237.47±7.38)μm2,(0.469 4±0.026 1),(0.599 6±0.030 2),(0.635 1±0.032 6)pixel.Group C,it was respectively(21.25±0.83),(14.63±0.90),(13.35±0.84)ms;(915±61),(1 146±36),(1 262 ±49)nV,(49.3±3.5),(60.6±3.3),(68.3±3.3);(231.81±7.38),(322.67±8.45),(386.82±10.42)μm2,(0.647 6±0.025 1),(0.761 1 ±0.030 5),(0.828 5±0.022 6)pixel].Compared with Group A,the difference was significant[Group A,the value was respectively(37.29 ±2.02),(31.07±2.06),(26.60±1.51)ms;(409±80),(667±66),(720±67)nV,(26.3±2.8),(31.0±3.0),(36.0±3.4);(98.12 ±4.93),(113.50±6.74),(122.59±8.03)μm2,(0.211 2±0.006 9),(0.277 3±0.011 7),(0.324 8±0.008 2)pixel]. In addition,the result of Group C was better than that of Group B.There was a significant difference. CONCLUSION:Tet can improve the microcirculation;relieve the secondary injury of tissues,protecting the experimental spinal cord. The combination of direct current stimulation with Tet produces a better result,especially in promoting the early recovery of nerves.
