摘要
目的观察BMP/α-TCP磷酸钙骨水泥在颅骨缺损修复中的作用。方法将BMP与α-TCP按比例制成BMP含量不同的BMP/α-TCP复合材料。分别采用α-TCP和BMP含量不同的BMP/α-TCP修复成年兔较大颅骨缺损。结果4周时可见BMP/α-TCP材料内大量类骨组织生成,8周时新骨量增加,16周骨缺损由新骨桥连修复。而α-TCP组8、16周时骨生成量和强度均低于BMP/α-TCP组(新骨量:BMP/α-TCP-A为46.81%,BMP/α-TCP-B为54.34%,α-TCP为22.96%,P<0.01)。结论BMP/α-TCP植入机体后可降解转化为新骨,且强度逐渐增加,适于颅骨及其他非承重性不规则骨缺损的修复重建。
Objective To investigate the the ability of new bone forming of a novel calcium phosphate bone cement loading bone morphogenetic protein(BMP). Methods Mixed BMP with the solid phase of α-TCP then with liquid phase. Two kinds of BMP/α-TCP cement(BMP/α-TCP-A contains 1% BMP, BMP/α-TCP-B, contain 2% BMP) were made thereafter. Three kinds of materials, including BMP/α-TCP containing1%,2% BMP and α-TCP cement alone were used to enhance the repairing of large skull defects in an experimental craniotomy model. Seventeen millimeter skull defects were performed in adult rabbit and were either implanted with α-TCP cement alone or BMP/α-TCP cement with two different doses of BMP(1%,2%). Results 8 or 16 weeks after operation, osteogenesis was insufficent to allow bone union when defects was filled with α-TCP cement alone. In contrast, addition of two diffent doses BMP toα-TCP cement led to a significant increase in osteogenesis and allowed bone union after 8 weeks.The fraction of new bone/connective tissue in defects filled with α-TCP cement alone,BMP/α-TCP-A and BMP/α-TCP-B were 22.96%/34.84%, 46.81%/5.12% and 54.34%/5.49% respectively 16 weeks after implantation and the BMP/α-TCP-B cement was the most potent activator of osteogenesis. However, the resorption of materials in α-TCP cement alone was almost the same as in BMP/α-TCP-B after 16 weeks but quicker than in BMP/α-TCP-A. Conclusions BMP/α-TCP cement was a novel combinted bioactive bone cement with osteotransduction and osteoinduction ability, the materials transformed to new bone after biodegradation in the body, with no decrease in compressive strength and volume loss, and maintained its self-setting characateristic under physiological condition. It is suitable for the repairing and reconstruction of cranial defects.
