摘要
目的 探讨超声引导下3 2 P胶体注射治疗甲状腺腺瘤和腺瘤囊性变的可行性和治疗效果。方法 采用稀释浓度为 37～ 74MBq/ 1 0～ 1 5ml和 18 5～ 37MBq/ml的两种注射液 ,按病变的体积调整剂量 ,分别对 38例甲状腺腺瘤和 30例腺瘤囊性变进行超声引导下3 2 P胶体注射治疗。 6 8例患者术前均经超声检查确诊 ,同期甲状腺扫描对照 ,病理穿刺证实。治疗前后均作血清FT3,FT4 ,TT3,TT4及TSH ,血常规检查 ,其结果进行对比。 32例随访 6～ 36个月评价其治疗效果。结果 甲状腺腺瘤治愈率 71 1% (2 7/ 38) ,腺瘤囊性变治愈率 86 7% (2 6 / 30 )。甲状腺腺瘤囊性变的治疗效果优于甲状腺腺瘤。治疗前后血常规、甲状腺激素检查结果对比差异无显著意义。所有病例均未出现明显副反应。结论 超声引导下3 2 P胶体注射治疗甲状腺腺瘤和腺瘤囊性变疗效显著 ,方法简便安全 。
Objective To evaluate the feasibility and effect of treatment of thyroid adenoma and cystic degeneration of thyroid adenoma by injection of colloid 32 P with ultrasonic guidance. Methods Diluted solutions of 32 P, 37～74 mBq/1.0～1.5 ml and 18.5～37 mBq/ml, were injected, with ultrasonic guidance, into the thyroid adenoma in 30 cases and degenerated cyst of thyroid adenoma in 30 cases, all confirmed by ultrasonography, thyroid scanning and pathologic paracentesis. The serum FT3, FT4, TT3, TT4, and TSH, and blood cells were examined before and after the treatment. Part of the patients was followed up for 6～36 months. Results The cure rate was 71.05% (27/38) for thyroid adenoma and 86.67% (26/30) for cystic degeneration of thyroid adenoma. The treatment effect was better for cystic degeneration of thyroid adenoma than for thyroid adenoma. The changes of blood cells and serum thyroid hormones were not significant before and after treatment. No obvious side effect was found in all patients. Conclusion Injection of 32 P with ultrasonic guidance is effective in treatment of thyroid adenoma and cystic degeneration of thyroid adenoma. This method is easy, safe and highly practical clinically.
