星形细胞瘤核形态测量的病理学分级及预后判断 被引量:1

THE ROLE OF NUCLEAR MORPHOMETRY IN PATHOLOGIC GRADING AND PROGNOSIS EVALUATING OF ASTROCYTOMAS

摘要 用全自动图像分析仪测量星形细胞瘤细胞核DNA含量及其倍体分布、核仁组成区嗜银蛋白(Ag-NOR)计数、核光密度变异系数及核的几何学参数,结果在病理分级方面,单因素分析时大多数参数与病理分级有关,但在多因素分析时,对组织学分级起主要作用的是核周长,其次为核积分光密度变异系数,2倍体及5倍体细胞百分比,核面积标准差也起重要作用。用8个参数建立起来的判别函数的回代符合率为83.7%,说明核形态测量对星形细胞瘤进行准确、客观的病理分级有实用价值,在预后方面,虽然DNA含量及某些核的几何学参数在单因素分析时有统计学意义,但在多因素分析时仅组织学分级、核平均光密度变异系数和Ag-NOR计数具有统计学的显著意义。 The nuclear features of 52 astrocytomas were studied by using automated image analysis system. The parameters included DNA ploid, Ag-NOR count, coefficient of variation of optical density, nuclear size and shape. The correlation between nuclear features, histologic grading and survival was studied. The important factors for grading were nuclear perimeter, coefficient of variation of integral optical density, the percentage of 20 and 50, and the standard deviation of nuclear area. The discriminatory function from 8 parameters was proved to be useful in grading. In prognosis, histologic grading and coefficient of variation of mean optical density and Ag-NOR count were independent indicators in multivariate analysis, although DNA content and some parameters of nuclear size and shape were statistically significante in univariate analysis. The result showed that histologic grading was the most important factor, and the coefficient of variation of mean optical density and Ag-NOR count were also independent prognostic indicators.
