螺旋CT及其多平面重建技术对判定食管癌侵犯周围结构的价值 被引量:16

摘要 目的 探讨螺旋CT多平面成像 (MPR)技术对判定食管癌侵犯周围器官结构的价值 ,并与横断面图像对比。方法 食管癌病例行前瞻性CT检查 ,43例有手术病理证实 ,其中行单层螺旋CT(SDCT)扫描 2 5例 ,多层螺旋CT(MDCT)扫描 18例。双盲法阅片。结果 横断面图像与MPR肿瘤侵犯判定的敏感性分别为 5 3 .85 % ,76.92 % ,特异性分别为 93 .3 3 % ,96.67% ,准确性分别为 81.40 % ,90 .70 % ,MPR优于横断面图像 ,但无统计学差异。结论 螺旋CT多平面重建图像能全面直观地显示肿瘤与周围结构的关系 ,对食管癌外侵判断的准确率较高。 Objective To evaluate the multiplanar reconstructions in the prediction of the apparatus invasion around esophageal carcinoma,and to compare with the transverse images. Methods Forty-three spiral CT examinations were performed in a prospective series,25 underwent SDCT,18 underwent MDCT.Two radiologists compared the transverse images with MPR images.Results In the evaluation of the neoplasitc invasion by transverse images and MPR images,the sensitivity,specificity and accuracy were 53.85% vs 76.92%,93.33% vs 96.67% and 81.40% vs 90.70% respectively.MPR was better than transverse images,but there was no statistical difference ( P >0.05). Conclusion The relationship between the neoplasm and adjacent apparatus could be displayed,and the accuracy was high in MPR images.
