摘要
目的报告小切口进行髋关节置换手术的可行性和优越性。方法自2001年9月~2004年5月,运用小切口对74例患者进行人工髋关节置换手术。结果切口长6.5~8.5cm。全髋置换平均手术时间80min,平均术中失血280mL,12h平均引流150mL;股骨头置换术平均手术时间40min,平均术中失血150mL,术后平均引流100mL。术后平均8d出院,平均住院14d。术后未发现并发症。随访1~32个月,复查X线片未见人工关节松脱,患者可负重行走。结论小切口具有创伤小、出血量少、损伤组织少、感染机会低、医源性损伤少、人工关节稳定性强、患者恢复时间短等优势。
Objective To report the feasibility and advantages of small incision in the hip replacements. Methods From September 2001 to May 2004, 64 cases of hip replacement through small skin incision were performed. Results The length of the skin incision ranged from 6.5 cm to 8.5 cm. The total hip replacements lasted from 75 to 120 mins (mean 80mins). The intraoperative blood loss was 200 to 450 mL (mean 280mL), and the drainage was 120 to 260 mL (mean 150mL) 12 hours after operation. The replacements of femoral head lasted 35 to 60 mins (mean 40mins). The intraoperative blood loss was 130 to 280 mL (mean 150mL) and the drainage was 80 to 150 mL (mean 100mL). The average time of discharge was 8 days after operation and the average hospitalization was 14 days. Neither local nor systemic complications had occurred. The follow ups of 1 to 32 months revealed that all the artificial hip joints maintained high stability by radiographic measurement and the patients could walk well without supports. Conclusions The traditional incision causes many obvious disadvantages, such as too much hemorrhage and larger wound. By contrast, the small skin incision leads to remarkable advantages: smaller wound, little hemorrhage, low infection rate, more stability of artificial joint and shorter recovering time.
