减重步行训练对脑源性步态异常患者步行能力的影响 被引量:10

摘要 目的:介绍减重步行训练对脑源性步态异常的治疗作用,并对临床效果做出初步的康复评定。方法:对21例脑卒中或脑损伤后所致步态异常患者应用减重步行训练为主的康复治疗,用功能性步行分级、Fugl-Meyer量表、Barthel指数评分分别进行康复评定。结果:21例患者经治疗后步态异常均取得明显改善,训练前21例患者步行分级均≤5级,训练后功能性步行分级为3级患者2例,4级患者7例,5级患者12例。生活质量明显提高(训练前60~41分患者7例,60~99分14例,训练后60~41分4例,60~99分11例,100分6例),差异具有显著性意义。结论:减重步行训练是一种安全、有效的治疗脑源性步态异常的方法,在脑功能损伤的康复治疗中有较广泛的应用前景。 AIM:To introduce the therapeutic effects of treadmill with body weight support(TWBWS) on brain derived gait irregularity,and evaluate the clinical effects on rehabilitation initially. METHODS:A total of 21 patients with gait irregularity induced by cerebral apoplexy or brain injury were given rehabilitative treatment mainly by TWBWS.All patients received rehabilitative evaluation with functional ambulation category(FAC),Fugl Meyer assessment and Barthel index respectively. RESULTS:Twenty one patients with gait irregularity were improved obviously after treatment including two patients in 3 degree of FAC,seven in 4 degree and twelve in 5 degree,while all patients were not more than 5 degree before treatment.In addition,the quality of life was improved significantly,the scores were 60 to 41 in seven patients and 60 to 99 in 14 patients before treatment,while 60 to 41 in four patients,60 to 99 in 11 patients and 100 in six patients after treatment.It was significant difference. CONCLUSION:TWBWS is a safe and effective method to treat brain derived gait irregularity and has a much prospect of rehabilitative treatment on brain function damage.
