摘要
目的 探讨母亲产前未用过激素的早产儿生后 12小时内预防性应用盐酸氨溴索针剂 (商品名沐舒坦 )早产儿肺氧合功能的变化。方法 早产儿 12 0例 ,按区组随机分为沐舒坦应用组和观察组各 6 0例 ,观察两组肺氧合及肺功能的改变 ,观察两组并发症发生率的情况。结果 (1)两组用药后 4 8h、 6 0 h、 72 h,肺氧合及肺功能指标差异有统计学意义 (P均 <0 .0 5 ) ;(2 )两组用药前及用药后 12 h、 2 4 h、 36 h,肺氧合及肺功能指标差异无统计学意义 (P均 >0 .0 5 ) ;(3)沐舒坦组支气管肺发育不良、脑室内出血及获得性肺炎发生率低于对照组 (P<0 .0 1)。结论 沐舒坦在用药后 4 8h始能改善早产儿的肺氧合及肺功能 ,显著减少与 NRDS相关并发症的发生率。
Objective To evaluate the pulmonary oxygen coalescent function change with ambroxol on prevention in premature babies whose mothers did not take prenatally corticosteroids.Methods Premature neonates were randomly divided into two groups:ambroxo group and control group,evaluate the pulmonary oxygen coalescent function change of the two groups and the related complications.Results (1)After being taken medicine,the data of pulmonary function and pulmonary oxygen coalescent function of thw two groups were significant difference(P<0.05)48 hours later,60 hours later,72 hours later.(2)Before being taken medicine 12 hours,24 hours,36 hours,after being taken medicine,the data of pulmonary function and pulmonary oxygen coalescent function of the two groups were not significant difference(P>0.05).(3)The incidence of premature-associated complications bronchopulmonary dysplasia(BPD),intraventricular haemorrhage,and post-natal acquired pneumonia were obviously different in the ambroxol group(P<0.01).Conclusion Ambroxol can improve the pulmonary function and pulmonary oxygen coalescent function of premature babies 48 hours later when being taken,and reduce the incidence of the related complications prominently.
