摘要
目的探讨翼状胬肉及其表面变性结膜广泛切除后制作上方相邻的带角膜缘干细胞的蒂状结膜瓣和下方相邻结膜穹窿部的蒂状结膜瓣相互滑行旋转移植治疗的效果。方法对98例(114眼)翼状胬肉患者,将翼状胬肉及其表面变性结膜广泛切除至泪阜根部及上下结膜穹窿底部,分别制作上方相邻健康的带角膜缘干细胞的蒂状结膜瓣和制作在泪阜根部及下方结膜穹窿底部的蒂状结膜瓣。将制作的两个蒂状结膜瓣滑行旋转进行移植。结果术后经6mo～3a的临床观察,翼状胬肉术后复发率为3.5%,愈合后局部平滑无皱折,眼球运动无影响。结论通过翼状胬肉及其表面变性结膜的广泛切除行带角膜缘干细胞的蒂状结膜瓣和穹窿结膜部蒂状结膜瓣滑行旋转移植,改变了翼状胬肉发病基础,而且应用自身健康的角膜缘干细胞和蒂状结膜瓣移植,血供良好,恢复快,不另取材,手术方法简单,是一种理想而且可以广泛适用的方法。
AIM: To evaluate the effect of implanting pedicle conjunctival flap with cornea limbal stem cells and that at fornix in treating pterygium.· METHODS: A total of 98 cases (114 eyes) underwent the implantation of pedicle conjunctival flap with cornea limbal stem cells and that at fornix.· RESULTS: During the follow-up of 6mo to 3a, the recurrent rate was 3.5%, the implanting part was smooth. Eyeball movement was not affected. · CONCLUSION: By implanting pedicle conjunctival flap with cornea limbal stem cells and that at fornix, the operation is simplified and patients' recovery is quickened. Thus it should be generalized in clinic.·
