摘要
目的 进一步提高胎儿面部畸形的超声检出率。方法 使用PHILIPSHDI 5 0 0 0SonoCT及Acuson 12 8XP彩色多普勒超声诊断系统 ,探头频率 2 .6~ 5 .0Hz ,对产前检查的 18~ 4 0周胎儿共 380 0例进行二维超声检查颜面部 ,其中 375例在二维基础上联合三维超声检查 ,选取胎儿面部有羊水的位置采集二维图像 ,再进行三维重建。结果 380 0例胎儿的鼻唇部检查中 ,二维超声能清楚显示 3344例 ,显示率 88.0 % ,其中 375例进行了二维和三维超声的联合检查 ,其三维显示清楚的有 30 1例 ,显示率 80 .3%。此 375例经联合检查后均能获得满意的胎儿鼻唇部图像。结论 二维、三维超声联合检查能更清晰、直观、准确地显示胎儿颜面部 (尤其是鼻唇部 )结构 ,大大提高了胎儿面部畸形 (唇腭裂 )的检出率。
Objective To raise the ultrasonic detection rate of fetus facial malformation.Methods 3 800 fetuses between 18 and 40 weeks' gestation underwent examinations by PHILIPS HDI 5000 Sono CT and Acuson 128 XP color Doppler ultrasound. Among them, 375 fetuses were subjected to three-dimensional (3D) ultrasound following good-quality two-dimensional (2D) ultrasound. Though 3D reconstruction, the sonograms of different sections in which we are interested were obtained. Results In the 3 800 cases, 2D gave adequate results in 3 344 cases with the rate of presentation being 88.0%. In the 375 cases subject to 2D and 3D ultrasound examinations, 3D gave adequate results in 301 cases with the rate of presentation being 80.3%. The 375 cases got satisfied images with 2D and 3D ultrasound. Conclusion The examinations of 2D in combinations with 3D ultrasound can more clearly, intuitively and exactly display the structures of the fetuses' faces and raise greatly the detection rate of fetus' facial malformation.
