摘要
目的 对比经腹超声 (TAUS)和经直肠腔内超声 (TRUS)对膀胱肿瘤的诊断价值 ,提高超声对膀胱肿瘤的诊断水平。方法 采用 TAU S和 TRU S诊断 32例膀胱肿瘤 ,结果与手术病理对比分析。结果 TAU S和 TRUS对肿瘤检出率分别为 84 .5 %和 96 .6 % ,后者略高 ,但二者比较差异无显著性 (P>0 .10 )。对肿瘤分期的准确率 ,TAU S和 TRU S各为 5 3.1%和 87.5 %。二者比较差异有显著性 (P<0 .0 5 )。TAU S彩色多普勒血流显像(TAU S- CDFI)和 TRU S彩色多普勒血流显像 (TRU S- CDFI)对膀胱肿瘤动脉血流显示率分别为 36 %和 88% ,二者比较差异有非常显著性 (P<0 .0 1)。结论 TRU S和 TAU S对膀胱肿瘤的检出率二者比较差异无显著性。但TRUS对膀胱肿瘤分期优于 TAUS,可为临床治疗决策提供重要依据。TRUS提高了 CDFI对膀胱肿瘤动脉血流的显示率 ,对于膀胱肿瘤的鉴别诊断有一定价值。TRU S与 TAUS相结合能够提高超声对膀胱肿瘤的诊断水平。
Objective Study on diagnostic value to bladder tumor by transabdominal ultrasound (TAUS) comparing with transrectal ultrasound (TRUS),in order to raise diagnostic level to bladder tumor with ultrasound.Methods Thirty-two bladder tumors were diagnosed with TAUS and TRUS,data so-obtained were compared with those obtained from surgery and pathology.Results Detection rates of the bladder tumor were respectively 84.5% and 96.6% in TAUS and TRUS,the latter was a little higher,but without significant difference between the two methods (P>0.10).The accuracy of tumor staging was respectively 53.1% and 87.5% by TAUS and TRUS,with a significant difference (P<0.05).The display rates were respectively 36% and 88% by TAUS-CDFI and TRUS-CDFI,with a very significant difference (P<0.01).Conclusion Though there is no significant difference in detection rates of bladder tumor between TRUS and TAUS,the tumor-staging by TRUS is superior to by TAUS,thus offering an important basis for therapeutic strategy.TRUS improved the display rate of color Doppler flow imaging (CDFI) on the bladder tumor′s arterial flow,so benefiting the differential diagnosis of bladder tumor.A combination of TRUS with TAUS can raise the ultrasonography′s diagnostic level to the bladder tumor.
