摘要
目的 :比较钛镍形状记忆合金环抱式接骨板与克氏针内固定治疗锁骨骨折的疗效 ,并讨论相关问题。方法 :回顾性分析手术治疗锁骨骨折 3 6例 ,其中 16例采用钛镍形状记忆合金环抱式接骨板治疗 ,2 0例采用克氏针内固定治疗 ,比较两种方法的治疗效果。结果 :钛镍形状记忆合金环抱式接骨板内固定组手术时间平均3 5min ,术中出血平均 5 0ml。骨折平均愈合时间 3 .5个月。 16例骨折全部愈合 ,1例延迟愈合。按照韩平良等锁骨疗效标准 ,优 13例 ,良 2例 ,可l例 ,优良率 93 .7%。克氏针治疗组平均手术时间 45min ,术中出血平均 3 5ml。骨折平均愈合时间 4个月。 3例延迟愈合。优 10例 ,良 4例 ,可 6例。优良率 70 %。并发症包括感染、畸形愈合、血管神经损伤、肩关节活动受限等。两组间优良率有显著差异 (x2 =3 .90 8,P <0 .0 5 ) ,在手术时间、术中出血、骨折愈合时间无显著差异 ,在感染、骨折延迟愈合、患者不适感有显著差异 (x2 =4.5 62 ,P <0 .0 5 )。结论 :钛镍形状记忆合金环抱式接骨板在锁骨骨折内固定中的疗效优于克氏针内固定 。
Objective: To compare the effect of surgical treatment for clavicle fracture by TINI shape memory alloy(SMA)encircleing bone bonding plate and Kirschner's wires,while discuss the related problem.Method:Retrospective analyzed surgically treated 36 cases clavicle fractures,16 cases in which were treated by TINI shape memory alloy encircleing bone bonding plate(SMAP),20 cases by Kirschner's wires,compared the effect of these two methods.Result:In SMAP group,mean operating time were 35min (30～60min),mean bleeding amount were about 50ml(30～80m1).Mean heal time were 3.5 months(3(～5)months).All 16 fractures healed,1 case suffered delayed union.According to the effect standard of Han Ping-liang,execllent 13 cases,good 2 cases,fair 1 case.The rate of excellent and good was 93.7%.In Kirschner's group,the mean operating time were 45 min(35～70min),mean bleeding amount were about 35ml(20～65ml).The mean heal time were 4 months(3～7months).Seventeen cases got primary union,3 cases suffered delayed union.Excellent 10 cases,good 4 cases,fair 6 cases,The rate of excellent and good was 70%.The complications include infection、mal-union or neurovascular problems poor shoulder founction.There was significant difference in rate of excellent and good (x^2=3.908,P<0.05),no differences between groups were found by operating time and bleeding amount and union time.There was only significant difference in infection,fractures delayed union,patients uncomfortble complain t(x^2=4.562,P<0.05).Conclusion:TINI shape memory alloy encircleing bone bonding plate(SMAP)is a good internal fixation for treatment of clavicle fractures.
