体外循环全身热灌注疗法对中枢神经系统的影响及护理 被引量:1

摘要 目的 探讨体外循环全身热灌注疗法对中枢神经系统的影响及其围手术期的护理对策。方法 采用美国FCM的TEMET1 0 0 0型体外循环全身热灌注治疗系统对 80例各类晚期恶性肿瘤及丙型肝炎患者进行全身热疗 ,观察经 1 2 0min高温 (4 2 .5℃ )平台期后 ,患者出现的相关中枢神经统系异常的临床表现 ,并通过动物实验 ,观察治疗高温 (4 2 .5℃ )下脑组织的病理损伤情况。结果 共有1 8例 (占 2 2 .5 % )患者在术后出现相关的中枢神经系统异常的临床表现 ;动物实验病理结果表现为神经元细胞的水肿 ,经过 2 4~ 4 8h后都可恢复。结论 全身热疗对中枢神经系统造成的损伤是轻微的、可逆的。加强做好围手术期的针对性护理 。 Objective To evaluate the influence and the peri-operation nursing countermeasure on Extracorporeal Whole Body Hyperthermia(EWBH) to the central nervous system. Methods 80 cases of terminal cancer were received EWBH. We collected the data of the abnormal central nervous system symptoms according to EWBH. On the other hand, we observed what was changed in normal tissue by pathology on the animal models. Results There were 18 cases occurred he abnormal central nervous system symptoms. The animal pathologic exam also showed that neutron edema was occurred , but the abnormal change can be recovery after 24 hours to 48 hours. Conclusions The damage of EWBH to the central nervous system is limited and reversible. It is important to strengthen the peri-operation nursing countermeasure for reducing the damage and the symptoms.
