摘要
目的 :评价多层螺旋CT(MSCT)诊断颅颈交界区畸形的价值。材料和方法 :对 12例临床确诊为颅颈交界区畸形行MSCT增强扫描后结合三维后处理 ,分析椎动脉 (vertebralartery ,VA)的异常走行形式。 12例均行MR检查 ,5例行数字减影血管造影。结果 :本组颅底交界区畸形共观察到 5类VA走行异常及发育异常 :( 1)走行于融合的骨结构中 17条 ;( 2 )走行于融合的寰枕部下方 6条 ;( 3 )开窗现象 1条 ;( 4 )距离齿突较近 2条 ;( 5 )发育不良 3条。MSCT观察椎动脉形态与DSA显示一致。结论 :MSCT观察VA与骨等结构关系准确、方便 ,在一定程度上可取代DSA有创技术。同时椎动脉第3段走行趋势可预测寰枕侧块融合的程度。
Purpose: To study the value of MSCT diagnosis in the Craniocervical Region Malformation. Materials and Methods: Twelve cases of aberrant/abnormal vertebral artery(VA) with bone malformation were performed by 3D-MSCTA and MRI. Five cases were examined by DSA among them. Results: Five types of aberrant vertebral artery were observed in those patients. Type I: Seventeen VAs were observed going through the fusion of the atlas and occipital bone. Type II: Six VAs going under the fusion of the atlas and occipital bone. Type III: One VA was fenestrated. Type IV: Two VAs were closed to the odontoid process. Type V: Three VAs were hypoplasia. The results of VA in CTA corresponded with those in DSA. Conclusion: MSCT is a convenient and noninvasive technique in observing VAs and bone structure of the craniocervical region malformation. To some extent, the route of third segment verterbral artery may suggest the degree of the atlanto-occipital fusion.
