摘要
目的 研究经皮中心静脉置管 (PCVC)在极低出生体重儿 (VLBWI)治疗中的应用。方法 19例VLB WI,出生体重 986 - 15 0 0g ,胎龄为 2 8- 32周 ,采用引导针穿刺上肢浅静脉后将中心静脉导管送入上腔静脉的PCVC方法 ,观察置管操作时间、成功率、导管保留时间及并发症发生率。结果 19例中 ,17例一次穿刺成功 ,成功率达89.5 %。操作时间 30 - 5 0分钟 ,导管保留时间为 16 3- 6 0 7小时 ,平均 349.9小时。静脉输葡萄糖的浓度及溶液的渗透压均可明显增高。 2例因穿刺部位感染、1例导管与输液泵连接处破裂而提前拔管 ;3例感染 ,感染发生率为15 .8% ,无血栓性并发症及死亡。结论 经上肢浅静脉行PCVC在VLBWI操作简单、成功率高 ,导管保留时间长 ,并发症少 ,PCVC是VLBWI静脉输液的最佳途径。
Objective Report the experience of clinical use of percutaneous central venous catheterization(PCVC) to therapy of very low birthweight infants (VLBWI).Methods 19 cases VLBWI which birthweights ranged from 986 to1500 grams and the gestational ages ranged from 28 to 32 weeks were proceeded PCVC.After inserted successfully the introducer needle into the forearm's superficial vein, the Epicutaneo- Cave-Catheters 2184.00 were slowly advanced through the introducer needle to the superior vena cava.To observe the procedure time of catheterization,the duration of inserted catheters and the complication frequences.Results The catheters were successfully inserted in 17 cases once.The procedure time of catheterization is about 30-50 minutes.The duration of catheters in situ for a cumulative 163-607 hours and average duration of 349.9 hours(14.6 days).The continuous infusion The infectious complications occurred in 3 cases.The heart rates,respiratory rates and blood pressure of the cases were in normal ranges during the catheter-inserted.Conclusion The PCVC is less complications, can be used safely to deliver total or partial parenteral nutritions to VLBWI over a prolonged period of time. [
