摘要
目的 探讨老年糖尿病伴或不伴脑梗死患者听觉认知电位P3 0 0 的变化特点及诊断价值。方法 采用听觉Cond序列刺激的诱发电位方法对 81例老年糖尿病患者 (其中 35例合并有多发性脑梗死 )与 30例健康老人进行测试。结果 (1)糖尿病组与对照组健康老人比较 ,前者P3 0 0 波潜伏期明显延长 ,P3 0 0 波波幅显著降低。 (2 )糖尿病伴脑梗死组与不伴脑梗死组比较 ,前者P3 0 0 波潜伏期明显延长 ,P3 0 0 波波幅显著降低。 (3)糖尿病组伴血管性痴呆亚组(VD)与多发性脑梗死亚组 (MI)比较 ,前者P3 0 0 波潜伏期更显著延长 (P <0 .0 0 1) ,VD组P3 0 0 潜伏期与简易智能化量表(MMSE)评分呈显著性负相关 (P <0 .0 0 5 )。 (4 )糖尿病伴抑郁组与不伴抑郁组比较 ,前者P3 0 0 波潜伏期明显延长 ,P3 0 0 波波幅显著降低。结论 老年糖尿病患者比正常健康组P3 0 0 波潜伏期明显延长 ,P3 0 0 波幅明显降低 ,老年糖尿病组中合并脑梗死时P3 0 0 波改变更为显著。因此P3 0 0 测试更利于血管性痴呆的早期诊断及疾病严重程度的观察。
Objective To investigate the characteristics of auditory cognitive potential P 300in elderly diabetics with or without cerebral infarction. Methods Eighty-one elderly diabetic patients (35 with multiple cerebral infarctions) and 30 elderly controls were tested with method of auditory Cond paradigm evoked potential. Results (1) There were significant differences in amplitude and latency of P 300between patient group and control group. (2) Significant difference in latency of P 300between sub-groups of patients with multiple cerebral infarction dementia(MCID) and those with multiple cerebral infarction(MCI) was also found(P<0.001). There was negative correlation between P 300latency and mini-mental state examination(MMSE) score in MCID sub-group(P<0.005). Conclusions The effects of latency delay and amplitude reduction of P 300were more obvious in elderly diabetics than in healthy elderly people, with cerebral infarction patients being much more obvious. The examination of P 300is beneficial to early diagnosis and evaluation of MCID.
