摘要
【目的】 应用双功能彩色多普勒超声技术 ,检测正常儿童颈动脉血管壁结构及血流动力学参数 ,旨在确立其正常值范围 ,为动脉粥样硬化早期诊断和儿童期干预提供正常参考数据。 【方法】 检测 10 0例健康儿童双侧颈总动脉、颈内动脉内径及内膜 -中层厚度 (IMT) ,颈总动脉分叉处内膜 中层厚度 (BIMT) ,颈总及颈内收缩期峰值流速 ,舒张末期流速。 【结果】 正常儿童IMT及BIMT分别为 :3~ 6岁组 ( 0 .3 9± 0 .0 4)mm和 ( 0 .42± 0 .0 3 )mm ,7~ 9岁 ( 0 .41± 0 .0 3 )mm和 ( 0 .44± 0 .0 6)mm ,10~ 12岁组 ( 0 .44± 0 .0 6)mm和 ( 0 .5 0± 0 .12 )mm ,13~ 15岁组 ( 0 .45± 0 .0 7)mm和 ( 0 .5 6± 0 .11)mm ,16~ 18岁组 ( 0 .48± 0 .0 4)mm和 ( 0 .5 9± 0 .0 9)mm。 16~ 18岁组与 9岁前组比较 ,各组间IMT及BIMT测值差异有显著性 (P <0 .0 5 ) ;与 10~ 12岁组比较BIMT测值差异有显著性 (P <0 .0 5 )。 【结论】 建议颈总动脉 (CCA)IMT及BIMT正常值 :10岁前分别为 <0 .45mm及 <0 .5 5mm。 18岁前分别为 <0 .5 5mm及 <0 .70mm。
To study the morphology and hemadynamics of the carotid artery in 100 healthy children with color Doppler ultrasound. Color Doppler ultrasound was used to determine the normal values. The internal diameter, intimal medium thickness(IMT) in carotid and bifurcation of carotid(BIMT), peak systolic velocity, and diastolic velocity were measured respectively. IMT and BIMT of normal children were listed respectively as:(0.39±0.04) mm and (0.42±0.03)mm in 3~6 years group,(0.41±0.03) mm and (0.44±0.06) mm in 7~9 years group,(0.44±0.06) mm and (0.50±0.12) mm in 10~12 years group,(0.45±0.07) mm and (0.56±0.11) mm in 13~15 years groups, (0.48±0.04) mm and (0.59±0.09) mm in 16~18 years group. There were great difference of the IMT and BIMT figures between 16~18 years group and 3~6 years group( P <0.05). [Conclusions] It is recommended to take the following figures as the normal IMT and BIMT of CCA: <0.45 mm and <0.55 mm respectively for children under 10 years, and <0.55 mm and <0.70 mm respectively for adolescents aged 11~18 years. An exceeding figure can be treated as the abnormal thickness of the carotid wall.
