摘要
目的探讨组织内激光凝固(ILP)治疗肺转移瘤的方法和疗效。方法应用Diomed半导体激光治疗24例肺转移瘤的病人,共计56个病灶。病灶大小0.8cm~3.6cm,其中≤2cm者38个,>2cm者18个,激光功率2w~10w,能量2400~12000焦耳,消融时间1200s。治疗后3个月复查CT评价治疗效果。结果转移病灶消失10个(17.9%),转移灶缩小35个(62.5%),转移灶不变或进展11个(19.6%),总有效率(80.4%),其中病灶≤2cm者有效率明显高于病灶>2cm者。全组1年、2年和3年生存率分别为75%、50%和33.3%。结论组织内激光凝固技术是治疗直径≤2cm的肺转移瘤的有效方法,其近期疗效与肺转移瘤灶的大小有关。
Objective To explore the effects and methods of interstitial laser photocoagulation (ILP) in the treatment of pulmonary metastases. Methods From 2001 to 2004, 24 cases with pulmonary metastases were treated by Diomed Laser. The total number of the focus were 56. The diameter of the metastases was 0.8 ~3.6 cm. Among of them, less than 2 cm in 38 and more than 2 cm in 18. The power of laser is 2 w and 10 w, The dose was 2400~12000 J. The time of ablation was 1200 S. CT scan was done after 3 months. Result Complete regression of the metastases was found in 10 foci (17.9%), dwindled in 35 foci (62.5%) and no change or enlarge in 11 foci (19.6%). The overall effective rate was 80.4%. Among of them , the effective rate for the foci with diameter less them 2 cm was 80.4%. The survival rate was 75%at 1 year, 50%at 2 years and 33.3 at 3 years. Conclusion Interstitial Laser photocoagulation is an effective therapeutic methods to pulmonary metastases.
